Objective:To compare the difference between simple decompression and decompression combined with fusion,â… ã€â…¡ clinical results with two kinds of surgical treatment for the true lumbar olisthe disease.Methods:According to the surgical method, 75 cases from â… ã€â…¡ of true lumbar olisthe hospitalized patients were divided into simple decompression (group A,35 patients) and decompression combined with fusion (group B, 40 patients) from two years between June 2008 and June 2010. We compared two different surgical methods that operation time and blood loss, after 3 months and 1year postoperative follow-up, we compared JOA score, VAS score, slip rate, slip Angle and intervertebral height change. Results:Group A had significantly less operation time and blood loss than group B, postoperative symptoms in 4 cases of group A were not obvious relief,2 cases followed for 3 months were relief, the remaining 2 cases were not obvious relief,4 cases followed for lyear that the degree of lumbar olisthe were aggravated and appeared the nerve root injury. Group B in the operation appeared 1 case of cerebrospinal fluid leakage and treated for suture repairing in the operation, also appeared 1 case of the nerve root injury. Comparing postoperative JOA score in two groups had no statistical difference after 3 months and 1year (P>0.05), VAS score for back and leg pain decreased obviously both at 3 months and 1 year in each group, the lumbago had no statistical difference in group A and group B after the operation of 3 month(P>0.05), and VAS score in group A compared with group B for lumbago had statistical significance at lyear follow-up(P<0.05).Both at 3 months andlyear, the group A through the lumbar spine X-ray check, the slippage rate, slip angle and intervertebral disc height were no obvious change. Both at 3 months and 1 year, the group B were in good condition,and the two group Postoperative slip rate,slip angle,intervertebral height changes,the differences were statistically significant (P<0.05).Conclusion:Both simple decompress- sion and decompression combined with fusion that â… ã€â…¡ clinical results with two kinds of surgical treatment for the true lumbar olisthe disease have no significant difference, the former has less surgical trauma, less time, less bleeding so that the better patient tolerance, decompression combined with fusion can reduce low back pain in patients with long-term recurrence rate and reduce variation in disc height, angle and slippage rates. |
