Objective: To investigate the safety and feasibility of gasless laparoscopic surgery in pregnant women with adnexal masses.Methods: A Retrospective study of 18 patients who had surgery for treatment of adnexal masses during pregnancy in the Second Affiliated Hospital of Soochow University,from Dec. 2011 to Dec. 2014.The operation time,bleeding volume,postoperative exhaust time, postoperative hospital stay, intraoperative and postoperative complications; and pregnancy outcomes after suegery were recorded and analysed by spss19.0.Results:18 patients with adnexal masses during pregnancy were underwent ovarian cystectomy including 11 cases with gasless laparoscopy,and 7 with laparotomy.Compared in the postoperative exhaust time and the postoperative hospital stay,gasless laparoscopy group was shorter than laparotomy group(p<0.05).When the adnexal mass diameter<100mm, the duration of operation and the bleeding volume had no significant difference(p>0.05). Compared in birth gestational age, birth weight, birth length and Apgar Rating(1 minute, 5 minutes), there was no significant difference(p>0.05).Conclusion:In our study,gasless laparoscopic surgery in pregnant women with adnexal masses did not increasing the perioperative complications, and pregnancy outcome had no significant difference.The operation time may slightly extended in gasless laparoscopic group, but it was trauma, less pain, and in the postoperative exhaust time and postoperative hospital stay significantly shorter than laparotomy.Compared with traditional laparoscopy,it did not using the establishment of CO2 pneumoperitoneum, had avoided the effect of CO2 and insufflation abdominal pressure, avoided adverse operations for pneumoperitoneum, and carried out in vitro to operating. Thus, gasless laparoscopic surgery in pregnant women with adnexal masses is safe, feasible,and worth being generalized for clinical use. |
