Background and Objective:In our country,gastric carcinoma,which derives from epithelial cells,is the most common malignant digestive tumor.At present,surgery is still the mainly treatment.Preoperative imaging evolution mainly concentrated in the shape and surrounding of the tumor,somewhat subjective.In clinic work,it is hard to find the difference between gastric carcinoma and high-risk gastric stromal tumor;Meanwhile,the selection of tumor treatment depends on its differentiation.So it is important to the comprehensive evolution of gastric cancer.The purpose of the study is to analysis the characteristic normal gastric wall,gastric carcinoma and gastric stromal tumor,and investigate the value of spectral CT imaging in diagnosis of gastric carcinoma.Materials and Methods:50 cases originating from stomach confirmed by pathology were analyzed retrospectively.There were 30 gastric carcinoma for the study group.20 gastric stromal tumor,also with 30 normal gastric wall were served as control group.30 gastric carcinoma were further grouped into two groups:13 cases as differentiated group and 17 cases as undifferentited group.All patients went through GSI imaging,after that,we took the arterial phase and venous phase images,at the same time,we got iodine based images and water based images.Afterward we analyzed the data.At last, independent samples T test was used to examine those datas.Results:(1)Parameter comparison between normal group and cancer group:both in arterial phase and venous phase,gastric cancer has higher value in CT values,the slope value is(2.41±0.47、2.72±0.57) vs(1.05±0.27、1.39±0.13);The iodine-concentration in two phases,cancer group(16.24±9.81、22.95±8.06) is higher than normal group(8.86±4.05、11.76±1.94);The effective-Z in two phases,cancer group(8.43±0.65、8.90±0.39) is higher than normal group(8.14 ± 0.24 、 8.31 ± 0.12);Differences of water-concentration between two groups was not found significant difference(P>0.05);(2)Parameter comparison between gastric carcinoma group and gastric stromal tumor:both in two enhancement phase,gastric cancer has higher value in CT values,the slope value is(2.41±0.47、2.72±0.57) vs(1.44±0.24、1.80±0.20);The iodine-concent in two phases,cancer group(16.24±9.81、22.95±8.06) is higher than stromal group(12.02±3.40、15.18±2.81);The effective-Z in two phases,cancer group(8.43±0.65、8.90±0.39) is higher than normal group(8.22 ± 0.19 、 8.41 ± 0.16);Differences of water-concentration between two groups was not found significant difference(P>0.05);(3)Parameter comparison between gastric cancer with different grades of differentiation:in venous phase,the undifferentiated tumors has higher CT values between 40~90kev,with higher slope value(4.02±1.52) than differentiated group(2.69±0.18). The iodine-concent and effective-Z in venous phase,undifferentiated group(27.30±4.51、9.12±0.24) is also higher than differentiated group(18.52±9.43 、 8.51±0.44).;Differences of water-concentration between two groups was not found significant difference(P>0.05).Conclusion:Spectral CT imaging has muti-parameter function,which can be helpful in diagnosis and differentiation of gastric cancer with iodine-concentration,the effective-Z and CT values.It has potential worth in evaluating the differentiation of the tumor,also offers a new idea and a new way of diagnosis.
