Application Of Dual-source CT Energy Imaging In Metastatic Lymph Nodes Of Gastric Carcinoma

Objective: To evaluate the value of dual source CT dual energy imaging in evaluating the peripheral lymph nodes of advanced gastric carcinoma.Methods: selected from January 2016 to June 2017 in our hospital,all patients suspected gastric cancer patients,collect all the inspection laboratory test results of these patients were performed one week before operation of Radiology,dual energy CT scan,without surgery were excluded,tracking and recording operation procedure,the annotation in operation finally,the lymph nodes,45 cases of lymph node metastasis confirmed by surgery and pathology in patients with gastric cancer,according to the pathological results of lymph node divided into gastric cancer cells invade lymph node(invasion group)and without lymph node invasion of gastric cancer cells(normal group),aortic iodine concentration measurement of gastric cancer lymph nodes the arterial and venous phase of the iodine concentration and the same level,calculated by the patients with gastric cancer gastric cancer cells of lymph node involvement and the normalized iodine concentration without gastric cancer cells invade the lymph node should be;Software analysis with SIEMENS dual energy CT postprocessing,respectively,two lymph node spectrum curve,analysis of gastric cancer cell invasion and lymph node without gastric cancer cells invade the lymphatic node spectrum curve(Energy Spectrum,Curve,ESC)slope characteristics.Results: the best single energy image is 70 keV.The NIC values of arterial phase in the invasive group and the normal group were respectively(0.545±0.087)and(0.143±0.078),the difference was statistically significant(P<0.05),and the venous phase was(0.478±0.014)and(0.306±0.083)respectively,the difference was statistically significant(P<0.05).The gastric cancer cell invasion and lymph node cells of gastric cancer without lymph node involvement of dual phase ESC were decreased,the arterial phase slope of k was 6.860,the diagnostic sensitivity of 83.24% gastric cancer cells from lymph node involvement,the specificity was 84.72%;the venous phase slope of k was 6.660,the sensitivity is 64.62%,specificity is 97.70%.Conclusion: dual source CT dual energy imaging is of certain value for the qualitative assessment of the peripheral lymph nodes of advanced gastric cancer.
