| Objective1. To investigate the psychological state (degrees of anxiety and dep ression) and the distribution of TCM constitution of HBV carriers. To stud y the relationship between TCM constitutional factors and HBV carriers’ psychological state. To study the effects of basic information (gender., ag e、course) and clinical examination indexes(such as quantification of HBV DNA、Fibroscan values)on patients’constitution and psychological state.2. To provide clinical data for establishing TCM clinic path for the HBV carriers by building up the database about the TCM constitution and psychological state of HBV carries, in order to control the progression o f HBV carriers.MethodsWe made investigations on HBV carriers who took examination in Shenzhen traditional Chinese medicine hospital for a year. The survey includes four parts as follows:part one is to collect basic information of HBV carries ; part two is to classify the TCM constitution of HBV carriers by using The Constitution in Chinese Medicine Questionnaire (CCMQ); part three is to asses the psychological state of HBV carriers by using Self-Rating Anxiety Scale(SAS) and Self-Rating Depression Scale(SDS); part four is to collect clinical examination indexes. All respondents should be informed the consent procedure before the research. Data were analyzed by SPSS 19.0 version statistic software. Chi-square test, ANOVA or Kruskal-Wallis test was used to analyze the the correlation between TCM constitution, psychological state and clinical examination indexes.Results1. The constitutional types of the 248 samples from high to low rankin -gs:Gentleness type(27.4%), Qi-deficiency type(11.7%), Wet-heat type(11.7%) ,Shen deficiency type (11.3%),Qi-depression type(10.9%),Phlegm-wetness ty pe(7.7%), Yin-deficiency type(7.7%), Yang-deficiency type(6.0%), Blood-stasi s type(3.6%),Special diathesis type(1.2%). The proportion of pathological constitution is very large(72.6%).The constitutional types of Non-active HBsAg carriers from high to low rankings:Gentleness type>Qi-deficiency type>Shen deficiency type>Yin-deficiency type>Qi-depression type>Phlegm-wetness type>Wet-heat type;The constitutional types of Chronic HBV carriers from high to low rankings:Ge-ntleness type>Wet-heat type>Qi-depression type>Qi-deficiency type>Shen d-eficiency type>Phlegm-wetness type>Yin-deficiency type=Yang-deficiency t ype> Blood-stasis type>Special diathesis type, It had no significantly dif-ference between the two groups (P>0.05).When the range of quantification of HBV DNA within 20IU/ml to 107IU/ml , the constitutional types from high to low rankings:Gentleness type>Shen deficiency type>Qi-deficiency type> Yang-deficiency type>Yin-deficiency t-ype=Qi-depression type>Phlegm-wetness type>Wet-heat type>Blood-stasis t ype>Special diathesis type;When the quantification of HBV DNA above 107IU/ ml,the constitutional types from high to low rankings:Wet-heat type)Qi-de-pression type>Phlegm-wetness type>Qi-deficiency type>Shen deficiency typ e>Blood-stasis type>Yin-deficiency type>Yang-deficiency type>Special diat hesis type. It had significantly difference between the two groups (P<0. 05).2. The mean and standard deviation of Self-Rating Anxiety Scale (SAS) score of HBV carriers was 52.24±10.10 points, the mean and standard deviation of Self-Rating Depression Scale (SDS) score was 53.92±10.11 points, both of them were higher than the normal range.According to level classification as≦49points、 50-59points、 60-69po ints、>69points, the SAS scores of HBV carriers can be divided into four pa rts:no anxiety(36.3%), mild anxiety(43.95),moderate anxiety (13.3%), seve re anxiety(6.45%),63.7% of HBV carriers have different levels of anxiety. According to level classification as≦49points,50-59points、60-69points、> 69points, the SDS scores of HBV carriers can be divided into four parts:n o depressive(35.49%), mild depressive(35.48%),moderate depressive(22.18%), severe depressive (6.85%),64.51%of HBV carriers have different levels o f depression.HBV carriers are divided into 3 groups by age as 18-29 years old,30-50 years old, above 51 years old. The depression psychology of HBV carriers at the age from 30 to 50 years old were more severe than the other groups. IN different levels of SDS, rates of 30-50 years old HBV carries from mild depressive to severe depressive were 50.0%,56.4%,76.5% (P<0.05)The Chronic HBV carriers’ scores of SAS and SDS were significant higher than the non-active HBsAg carriers’(P<0.05). When the quantification of HBV DNA above 107IU/ml, HBV carriers’ scores of SAS and SDS were significant higher than the HBV carriers’,whose quantification of HBV DNA were within 20IU/ml to 107IU/ml (P<0.05).3. Compared the SAS scores of different HBV carriers’ constitutions:the other nine pathological constitutions’SAS scores were significant higher than the Gentleness type’s(P<0.05 或 P<0.01), Wet-heat type and Yin-deficiency type were higher than Qi-deficiency type,Yan-deficiency type, Blood-stasis type and Shen deficiency type. Compared the SDS scores of different HBV carriers’constitutions:the other 9 pathological constitutions’ SDS scores were significant higher than the Gentleness type’s(P<0.05 or P<0.01), the score of Qi-depression type was significantly higher than the other 8 pathological constitutions, except the Special diathesis type(P<0.05).The Logistic Regression showed that the values OR of mild anxiety a nd mild depressive were significantly higher in nine pathological constit utions than Gentleness type. In the level of mild anxiety, the values OR of Phlegm-wetness type, Qi-depression type, Wet-heat type and Yin-defic iency type were increased, which were 90.0,32.3,30.0 and 30.0, Phlegm-wetn ess type was the highest one;In the level of mild depressive, the value s OR of Yang-deficiency type, Phlegm-wetness type and Qi-depression type w ere increased, which were 36.27、36.27、and 25.909, Yang-deficiency type w as the highest one.4. Fibroscan values(FS values)of HBV carriers older than 30 years ol-d were significant higher than the HBV carriers younger than 30 years ol-d (P<0.00);FS values can be divided into four levels as FF1 term, F2 term ,F2F3 term,F3F4 term, according to the values:<7.3K/Pa,7.3-9.7K/Pa,9.7-12. 4K/Pa,>12.4K/Pa. The course in F2 term was much longer than F0F1 term (p<0. 05).The Chronic HBV carriers’FS values were significant higher than the non-active HBsAg carriers’;when the quantification of HBV DNA above 107I U/ml, FS values were significant increased(P>0.05)Compared the FS values of different HBV carriers’constitutions:Bloo d-stasis type was significant higher than the other eight pathological co nstitutions and the Gentleness type,Qi-deficiency type, Wet-heat type and Yin-deficiency type were higher than the Gentleness type(P<0.05).ConelusionsThere are internal relationships between TCM constitution, psychological state, basic information and clinical examination indexes of HBV carriers. To provide clinical data for establishing TCM clinic path for the HBV carriers by classifying the TCM constitution, assessing the psychological state and clinical examination. |
