| Objectives:To understand job burnout of midwives nowadays, analyse its modifying factors, find the root causes, thereby to provide a basis for finding pertinent and effective solutions, reducing loss of midwives, maintaining their psychological health and improving the quality of nursing practice in obstetrics.Methods:Using stratified cluster sampling, stratify the hospitals by their classes.Choose 3 class-3 and 10 class-2 hospitals in Baoding randomly. Then trained professionals did a survey among 206 qualified midwives in these 13 hospitals. The surveys include standard questionnaire, Maslach Burnout Inventory, MBI in short, which is designed by American psychologist Maslach and Jackson in 1986 and the commonly used SSRS in China.The subjects were asked to fill out the forms after agreement of the hospitals and subjects themselves.The investigators would first explain the purpose, method, matters needing attention and requirements of the survey.Then the forms were filled out anonymously and collected immediately upon finishing. All the figures were analysed by SPSS 19.0 statistical software after proof checking. Quantitative data were represented as.The two groups were compared and tested by independent sample t.One factor analysis was conducted by variance and linear correlation analysis method while multiple linear regression analysis was used for multi-variable.Results:The job burnout of midwives in the research of three dimension scores were compared to the job burnout scores of Ye Zhihong 1320 hangzhou nurses group(hereinafter referred to as the norm). Emotion exhaustion scores varies from 12 to 50, average(36.57±7.51),there were statistical differences comparing to normative score(t=20.164, P<0.05); depersonalization scores between 4 and 26, average(12.46±3.91), there were statistical differences comparing to normative score(t=13.726,P<0.05); self-fulfilment scores were between 22 and 45, average(31.31±4.56), there were statistical differences comparing to normative score( t=9.658,P<0.05)and subjects are all highly languid.Analysis of factors affecting midwives’ emotion exhaustion scores: age(F=19.104,P<0.05), title(F=26.900,P<0.05), education background(F=5.917,P<0.05), marital status(F=7.191,P<0.05)and years of service(F=19.775,P<0.05)are all influencing factors.Higher emotion exhaustion scores are found to be associated with older age, higher technical title, lower education background, marriage, longer years of service and vice versa. Besides, linear correlation analysis shows that emotion exhaustion(dependant variable) is linear with social support(independent variable). The higher of social support scores, the lower score of midwives’ emotion exhaustion(r=-0.209,P=0.003). Multi-factor analysis demonstrates that age, education background, technical title and social support are influencing factors of midwives emotion exhaustion(adopt between 0.05 and 0.10). From standard partial regression coefficient’s point of view, title is the most influential factor, followed by age and social support.Analysis of factors affecting depersonalization on Midwives job burnout: single factor analysis indicates that, age( F=8.199,P < 0.05), title(F=6.115,P<0.05), education background(F=3.084,P<0.05), marital status(F=12.891,P<0.05)and years of service(F=6.837,P<0.05)are all influencing factors.The older age, higher title, longer of working years, the lower is the score.Married midwives score higher than single ones while midwives with a higher education background scores less.Moreover, linear correlation analysis shows that the score of depersonalization(dependant variable) is not linear with social support(independent variable)(r=-0.060,P=0.407), there is no relationship between social support and depersonalization.Multiple factors analysis results indicates that the fixed number of year of midwifery(t=2.268,P<0.05) and marital status(t = 2.124, P < 0.05) is main influence factors to the personality tendency 0f midwife.Analysis of factors affecting self-fulfilment scores in Midwives job burnout: single factor analysis indicates that, age(F=8.942,P<0.05), title(F=12.809,P<0.05), marital status(F=23.937,P<0.05)and years of services( F=16.620,P < 0.05) are all influencing factors. Personal accomplishment score(dependant variable) is linear with social support(independent variable)(r=-0.212,P=0.003).Social support is associated with the sore of personal accomplishment.Multi-factor analysis demonstrates that title marital status and social support are accepted by the model.Subjects with higher technical title scores less(t=3.262,P<0.05),the lower level of social support the lower score of personal accomplishment(t=2.562,P<0.05), married midwives score lower than those are single(t=2.237,P<0.05).Conclusions:1 There is high level of job burnout among midwives. The survey indicates that, midwives job burnout is not only reflected in emotion exhaustion and decrease in self-fulfilment, but also in severe depersonalization, which warns that midwives job burnout could not be neglected and the departments should take positive measures.2 Age, technical title, education background, marital status and years of services are all affecting factors on job burnout scores. Midwives wither old age, high title, long working experience has severe job burnout and they should be paid more attention and effort to by nursing management departments. For high qualification midwives, there should be reasonable performance examination, job setting, and career planning to improve their morale, thereby to employ their advantages.3 Social support also influences midwives job burnout. There is negative correlation between social support and job burnout as well as personal accomplishment.The effect of social support on midwives job burnout must be emphasized. |
