| Objective: The aim of this study was to analyze the expression and evaluate clinic diagnostic value of tumor markers CEA, SCC, CA125, NSE, CYFRA21-1 and TK1 from serum in lung carcinoma.Method: Blood was took from 69 patients with lung cancer group, 40 control group(including 20 benign and 20 Normal physical examination) who were admitted to the Fourth Hospital Affiliated to Hebei Medical University in Shijiazhuang between January 2014 and August 2014. TK1 of serum was detected by enhanced chemi-luminescence assay, and the levels of CEA, SCC, CA125, NSE, CYFRA21-1 were detected by electrochemiluminescence immunoassay.Result:1 The levels of serum TK1ã€CYFRA21-1ã€CAl25ã€CEAã€SCC were significantly increased in lung cancer group(2.69±3.17 pmol/L, 5.44±8.14 IU/ml, 37.26±27.46 U/ml, 27.25±50.27 U/ml, 8.09±11.5 ng/ml, 2.30±3.91 U/ml, respectively) compare with control group(0.95±0.80 pmol/L, 1.84±0.55 IU/ml, 10.55±6.03 U/ml, 12.51±1.74 U/ml, 2.26±1.40 ng/ml, 0.65 ± 0.22 U/ml, respectively). However, the levels of serum TK1 ã€CYFRA21-1ã€CAl25ã€NSEã€CEAã€SCC were not significantly different in benign pulmonary disease group(1.05±1.28 pmol/L, 1.55±0.61 IU/ml, 17.8±15.28 U/ml, 16.2±4.97 U/ml, 1.63±1.10 ng/ml, 0.60±0.23 U/ml, respectively) compare with control group.2 The area(from small to large order) under the receiver operator characteristic(ROC) curve is a well established measure for determining the efficacy of CYFRA21-1,NSE,CEA,SCC,CA125,TK1(0.714, 0.716, 0.784, 0.834, 0.882 and 0.917, respectively), and the critical values(2.74ng/ml,14.2ng/ml,3.03ng/ml, 19.35u/ml, 1.245pmol/L and 1.05ng/ml,respectively) are less than the reference range. The area under the ROC of CA125 and SCC are more than 0.8, and the area under the ROC of TK1 is more than 0.9(the sensitivity was 98.6%, the specificity was 87.2%), which have high clinic diagnostic value of lung cancer.3 Expression of serum CEAã€CAl25ã€SCCã€NSEã€CYFRA21-1 were significantly different in SCLC(2.28±1.29 ng/ml, 25.71±11.95 U/ml, 1.07±0.56 U/ml, 73.82±80.57 U/ml, 2.03±1.50 IU/ml, respectively) compare with NSCLC(9.47±12.42 ng/ml, 39.97±29.37 U/ml, 6.24±8.85 U/ml, 17.35±8.64 U/ml, 6.24±8.85 IU/ml, respectively). However, the level of serum TK1 was not significantly different from SCLC(4.17±6.65 pmol/L) and NSCLC(2.22±1.06 pmol/L).4 The expression of serum CEAã€CAl25ã€SCCã€CYFRA21-1 were significantly different in squamous cell carcinoma group(14.04±14.81 ng/ml, 51.99±31.97 U/ml, 0.60±0.23 U/ml, 2.93±2.27 IU/ml, respectively) compare with adenocarcinoma group(3.29±1.81 ng/ml, 23.74±14.39 U/ml, 1.11±0.50 U/ml, 10.71±12.71 IU/ml, respectively). However, serum TK1 and NSE was not significantly different from squamous cell carcinoma(2.12±0.78 pmol/L, 17.45±7.61 U/ml, respectively) and adenocarcinoma(2.29±1.23 pmol/L, 18.10±10.49 U/ml, respectively).5 Positive rate of serum NSE was higher than other tumor markers in SCLC(54.5%); the positive rates of serum CYFRA21-1 and SCC were most high in squamous cell carcinoma of lung(70%, 65%, respectively); and the positive rates of serum CEA and CA125 were most high in adenocarcinoma of lung(44.4%, 62.9%, respectively).6 TK1 is a good cancer marker for early diagnosis of lung cancer that the sensitivity(44.93%), specificity(92.5%), and effectiveness(62.39%) were better than other tumor markers. The combination of TK1 with CEA, SCC, CA125, NSE, CYFRA21-1 significantly improve diagnosis in lung cancer, and the sensitivity was 87.0%.7 The detection combination of TK1 with NSE can improve sensitivity in SCLC; the combination of TK1, CYFRA21-1 and SCC was the best tumor marker for diagnosis of squamous cell carcinoma, and the combination of TK1, CEA, CA125 and CYRFA21-1 was the best tumor marker for diagnosis of adenocarcinoma.Conclusion: TK1 is a good cancer marker for early diagnosis of lung cancer, but no auxiliary diagnosis for pathological type of lung cancer. The combination of CEA, CA125, SCC, CYFRA21-1 and NSE was the best tumor marker for early diagnosis of lung cancer, and help for pathological type diagnosis of lung cancer. The combination of TK1 with NSE; TK1, CYFRA21-1 with SCC; and TK1, CEA, CA125 with CYRFA21-1 were the best tumor marker for diagnosis of SCLC, squamous cell carcinoma and adenocarcinoma, respectively. |
