Objective: To study visual quality in patients with bilateral monofocal intraocular lenses monovision.Methods: Prospective nonrandomized controlled study.This study comprised consecutive bilateral age related cataract patients 64 cases(128eyes) between December 2013 and December 2012. All cases were divided into two groups : monovision group(31 patients) and conventional group(31 patients).All the patients enrolled were implanted the same type of IOL(AMO Tecnis ZCB00).In monovision group,the target refraction ranges of dominant eye was -0.5D~ 0D,and -2.0D~-1.5D in nondominant eye. In conventional group,the target refraction ranges of both eyes were -0.5D~0D.Parameters were analyzed included binocular and monocular uncorrected distance, intermediate, and near visual acuities in 1 week、1 month and 3 months postoperatively. Parameters were analyzed included binocular and monocular uncorrected distance, intermediate, and near visual acuities3 months postoperatively. Assessed were refraction state、Chinese reading visual acuity and stereopsis acuity 3 months postoperatively. Parameters analyzed 3 months postoperatively included subjective visual function and spectacle independence.Data processing and statistical analysis used SPSS16.0 statistical analysis package, t test was used to compare the measure data which met normal distribution, the chi-square test was applied to compare counting card information,and the rank sum test was used to compare the ranked data.There is significant difference between the two groups(P<0.05).Results:(1) Mean binocular uncorrected distance visual acuity was 0.035±0.052 (logMAR) and 0.036±0.056(logMAR) for monovision group and conventional group respectively(p> 0.05). (2) Mean binocular uncorrected intermediat visual acuity was 0.349±0.097 (logMAR) and 0.608±0.079(logMAR) for monovision group and conventional group respectively (p<0.05). (3)Mean binocular uncorrected near visual acuity was 0.366±0.096 (logMAR) and 0.626±0.064(logMAR) for monovision group and conventional group respectively(p<0.05). (4) Mean chinese reading visual acuity was 9.29±2.64 pt and 16.92±4.38pt for monovision group and conventional group respectively(p<0.05).(5)The far stereopsis acuity between monovision group and conventional group had statistical significance (p<0.05).But there were no statistically significant difference in far stereopsis acuity with corrected distancer visual acuity between two groups(p>0.05). (6)The total scores about examination of visual function scale was 79.31±3.52 and 73.25±3.76 for monovision group and conventional group respectively(p<0.05).The items’scores of daily life’s limitation involving with vision in monovision group is better than conventional group(p<0.05),but conventional group is better than monovision group in far stereopsis acuity(p<0.05).Two groups have no statistical significance in vision of peripheral field sensory adaptation(p>0.05).(7)The rate of spectacle independence was 83.9% and 12.1% for monovision group and conventional group respectively.Two groups have statistical significance(p<0.05).Conclusion:Monovision pseudopphakia can obtain good UCDVA、UCIDV、UCNDV and chinese reading visual acuity,improve the rate of spectacle independence,but reduces the far stereopsis acuity.
