ObjectiveStudy of viral myocarditis in children with ventricular late potentials and heart rate variability characteristics, to explore the value of ventricular late potentials and the indexes of heart rate variability on children with viral myocarditis clinical diagnosis, treatment and prognosis. MethodsThe object of study:268 children hospitalized with viral myocarditis collected from January 2012 to October 2013 included. Age from 7 to 15 years old, average age were 13±3.4 years old. male 159 and female 109. Control group : Selected the healthy children who were tested in hospital at the same time. All of them 189, age from 6 to 16,average age were 13±3.9 years old.All of them did not take any medicine that effects of autonomic nervous activity before examination and had no effect of autonomic nervous activity history. The two groups are matched in age and gender.Diagnostic criteria:All of them were definitive diagnosed viral myocarditis in line with the standard which Chinese Medical Association Medical Pediatric Branch Group of Cardiology Editorial board of Chinese Journal of Pediatrics Clinical criteria for the diagnosis of viral myocarditis that Enacted in 1999. All of them were in acute stage.Selection of indicators are as follow : The Ventricular Late Potentialn indexes: TQRS, LAS40 RMS40. HRV time domain indexes SDNN, SDANN, r MSSD, PNN50; HRV frequency domain indexs : LF, HF, LF/HF.Statistic process :All data using SPSS13.0 statistical software for statistical analysis. Measurement data using mean ± SD, Measurement data is compared between groups using t test, p< 0.05 means the difference was statistically significant. Results1. The TQRS and the LAS40 VMC group of ventricular late potential index values compared with the control group, significantly higher, the differences were statistically significant(P<0.05); VMC group compared with control group, RMS40 value, the difference was not significant, no statistical significance(P>0.05)2. The VMC group compared with the control group, the indexes of 24 h DCG time domain index of heart rate variability and frequency index values of SDNN, SDANN, r SMMD, PNN50 and LF, HF, LF/HF and control group at each age were decreased, the difference has statistical significance(P<0.05). Conclusions:1. The emergence of ventricular late potentials, and inflammatory injury induced by myocardial ischemia and myocardial lesions result in ventricular repolarization inconsistencies, may cause arrhythmia and sudden death; the reduces of heart rate variability index, suggesting impaired autonomic function, vagus nerve dysfunction, sympathetic hyperfunction of function, regulation of the sinoatrial node, may easy the rhythm arrhythmias and sudden death.2. Combined with the two indexes of ventricular late potentials and heart rate variability, and corresponding clinical symptoms comparison, found that the combination of two indicators of viral myocarditis, early diagnosis, treatment and prognosis, and is an important index to judge the severity degree and danger.
