Objective:To compare the clinical therapeutic effects of operation via the three incisions of right chest and two incisions of left breast in treatment of the middle segment esophageal squamous cell carcinoma.Method:The 97 cases of esophageal cancer patients who accepted surgical treatment in our hospital from December 2006 to December 2008 were selected as the sample. The preoperative and postoperative pathology are both confirmed as esophageal squamous cell carcinoma. The cases include 55 cases of the right chest surgery path (observation group) and 42 cases of the left breast surgery path (control group). The postoperative drainage, surgical complications, and the condition and survival rate of lymph node resection in the two kinds of surgery paths were retrospectively and statistically analyzed.Results:Two patients in observation group died of anastomotic fistula and postoperative respiratory function failure and no patient died in control group.Most of the occurrence rates of surgical complications in the two groups complications are no difference, but the lung infection and incidence of arrhythmia of the observation group is significant (P<0.05); The observation group cleaned 990 lymph nodes and the positive rate of lymph node is 23.8%, but the control group cleaned 504 lymph nodes and the positive rate of lymph node is 12.4%(P<0.05); The five-year survival rate (45.5%) in the observation group is higher than the rate (21.4%) in the other one (P<0.05); The observation group operative time and intraoperative blood loss compared with the control group has significant difference (P<0.05),but the drainage of the postoperative first day no significant difference.Conclusions:If the esophageal cancer patients have a good situation, and unable to determine whether the mediastinum and neck lymph nodes have transfer, selective right chest incision surgery is a good way.
