| Objective1. The study explored the common Traditional Chinese Medicine constitution syndromes in patients with hyperuricemia. The main constitution syndromes of Traditional Chinese Medicine were intervened with compound Tufuling Granules. The constitution scores and the levels of serum uric acid were evaluated before and after treatment. After that, we explored the correlations among serum uric acid levels, constitution syndrome and other possible factors, for providing clinical data to support good effect of treatment on hyperuricemia by Chinese medicine.2. Coping the hyperuricemia rats of syndrome of phlegm-dampness, we observed the impact of compound Tufuling Granules to this model including its Traditional Chinese Medicine constitution clinical symptoms, serum uric acid, liver function, blood lipids, liver pathology and protein levels of renal urate transporter 1 (mURATl). The test validated the effectiveness of compound Tufuling Granules treating on hyperuricemia by basic research, and initially explored a part of its mechanism, the trial provided a theoretical reference for its clinical application.Methods1. We collected 150 patients with hyperuricemia in the clinical research, there were 92 patients actually in the finally, they were divided into three Groups according to the different constitution, there were 36 patients with phlegm-dampness constitution,32 patients with yang-deficiency constitution,30 patients with qi deficiency constitution. Then each was educated health and given compound Tufuling Granules. One courses was 20 days. We inquired and recorded thegeneral condition of the patients before the treatment, improved the laboratory examination, then reviewed serum indicators and constitution score after treatment. Statistics was made by self control, comparison between groups and correlation ananlysis.2.48 rats were randomly divided into blank control group, model group, comparison group, compound Tufuling Granules(low, mid, high)-dose group, all groups except blank control group were been made to hyperuricemia rat of syndrome of phlegm-dampness which were fed with high fat fodder, taken hypoxanthine by intragastric administration, made Oxygen oxazine acid potassium through Intraperitoneal injection, and then were given appropriate treatment;Detected the variation of behavioristics, and serum uric acid, alanine aminotransferase, aspartate-aminotransferase, triglyceride, low density lipoprotein, protein levels of mURAT1 and liver pathology of each rat in 30 days.Results1. The significant decrease of the serum uric acid levels and constitution scores were found in the different groups of patients after treatment(P< 0.05). The decrease of the degree of the serum uric acid levels and constitution scores in the group of phlegm-dampness constitution more than the group of yang-deficiency and qi deficiency (P<0.05).2. The results of partial correlation showed that the level of serum uric acid has positive correlation with constitution scores, gender, body mass index, high-purine diet, genetic history, low density lipoprotein, triglyceride about patients with phlegm-dampness correlation. The level of serum uric acid had negative correlation with systolic blood pressure. The level of serum uric acid is intimate relativity with constitution scores, body mass index, genetic history, low density lipoprotein, triglyceride.3. The result of multiple stepwise regression analysis suggested that phlegm-dampness constitution have more impact on the serum uric acid levels,both have positive correlation, but the systolic blood pressure have negative correlation with the serum uric acid levels, and both don’t have collinearity problem.4. All groups didn’t appear adverse reaction after treatment, there was no statistically significant difference during about liver function and renal function (P>0.05). A few patients with abnormality of liver and kidney function could reduce to normal.5. The mice of pass counts and vertical counts in the model group were less than which in the blank group, and statistically difference was significant (P <0.05). Pass counts and vertical counts of the rats of in high and middledose groups of the compound Tufuling Granules were more than which in the model group. There was statistically difference(P< 0.05), and their mental condition, hair color, appetite, faeces and urinate improved obviously. But there were no significant difference between the comparison group and the model group (P>0.05).6.The levels of serum uric acid in model group were significantly higher than blank control group(P<0.05). The levels of serum uric acid in high and middle dose compound Tufuling Granules groups were significantly lower than model group, there was statistically difference(P<0.05), and compound Tufuling Granules high-dose group reduced more obviously when compared with the blank group.7. Compared to rats in model group, the expression level of mURAT1 protein of rats in comparison group was more lower (P<0.05). Compound Tufuling Granules high and middle dose group treatment could significantly inhibited the expression levels of mURATl protein, compared with the model group(P<0.05).8. The levels of alanine aminotransferase, aspartate aminotransferase, triglyceride and low density lipoprotein of rats in model group were significantly higher than control group (P<0.05). The levels of alanine aminotransferase aspartate aminotransferase, triglyceride and low density lipoprotein of rats in high-dose compound Tufuling Granules group were validly reduced, compared to these in model group(P<0.05).The hepatic tissue pathology of rats in all groups was not significantly anomal. The hepatic tissue of rats inmodel group appeared a few inflammatory cells infiltration and hepatocyte necrosis of individual liver cells.Conclusion1.The common constitution syndromes are phlegm-dampness syndrome in hyperuricemia, followed by yang-deficiency and qi deficiency. The main of annexing constitution are phlegm-dampness and qi deficiency syndromes.There is close correlation between uric acid levels and constitution scores, they have positive correlation. Compound Tufuling Granules may decrease validly uric acid levels and improve the phlegm-dampness constitution in patients with hyperuricemia.2. Compound Tufuling Granules may reduce the level of serum uric acid of in Hyperuricemia rats with phlegm-dampness, improve constitution of phlegm-dampness, liver function and blood lipids is quitely effective. The mechanism of reducing serum uric acid may be involved in down-regulation of protein of renal mURAT1. |
