| ObjectiveThe purpose of the observation and comparison of the clinical effects of lumbar disc herniation(LDH) of acute stage between Warm Needling working with Electroacupuncture and pure Electroacupuncture is to provide an idea and reference for the treatment of acute lumbar disc herniation.MethodsThis study was based on the diagnostic criteria and standards of TCM the lumbar disc herniation issued by The Ministry of Health of the People’s Republic of China in 2002. Under the voluntary principle, we have selected 60 patients, who are being diagnosed as the acute stage of LDH from December 2013 to January 2015. These cases come from The Acupuncture Department and Spine Surgery Department of the NO.1 Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine. The patients were randomly divided into 2 groups:treatment group and control group, respectively each 30 people. The treatment group-was treated with Warm Needling and Electroacupuncture, while the control group treated with only Electroacupuncture.(1)The Treatment GroupAcupuncture on paravertebral point on Lesions Lumbar disc segment and the upper and the lower, GB30, BL40, GB34, BL65, GB41 of the affected side. Giving paravertebral point on electric stimulation,using disperse-dense wave, frequencies of 2~24hz, then put the ignited moxa-stick on the neddles (the burning side face to the skin), burning two sticks, TDP irradiation on the waist, lasting 30mins. Once per day, six times per week as one treatment period. Take 2 treatment period.(2)The control GroupOnly Electroacupuncture treatment, the treatment operations and courses are the same as the treatment group.(3)Daily CareBoth groups are suggested to take cold protection, prevent raw and cold food, rest on hard board, and activities under protection of lumbar corset.(4)Cases Collected ContentPatients’age, sex, course of disease, lesion segment, the score charges of visual analogue scale (VAS) on 5 times, that is before treatment, after the first treatment, after the third treatment, after one treatment period and after all treatment; Oswestry functional disability index score on before and after treatment, lower back pain JOA score on before and after treatment, evaluation of the efficacy.(5)Statistical analysisUse SPSS19.0 statistical analysis Package to analyze the data.Results(1) Among 60 patients,26 cases have been backache,28 cases has Multi-segment disc herniation,15 cases L4/5,17 cases L5/S1. Multi-segment disc herniation is the most.(2) After the first time treatment, the VAS scores is significantly difference between two groups (P<0.05). According to this, in treating Lumbar disc herniation of acute stage, the Warm Needling and Electroacupuncture group’s immediately analgia effect is better than the Electroacupuncture group.(3) Both the two groups could reduce the Oswestry scores and the VAS scores, and raise the lower back pain JOA scores. The significant difference of the points was found between pretreatment and posttreatment(P<0.01). According to this, both treatment can relieve pain and improve the activity function of LDH of acute stage.(4) After all the treatment, the lower back pain JOA, the VAS scores, Oswestry scores between the two groups have significant differences (P<0.05), which showed that the effect of Warm Needling add Electroacupuncture on the acute stage of LDH is better than pure Electroacupuncture.(5)statistics of curative effect, the treatment group are clinic cured in 2 cases, marked effect in 22 cases, effective in 4 cases and no effect in 2 cases. The toal effective rate is 93.3%. The control group are clinic cured in no cases, marked effect in 11 cases, effective in 15 cases and no effect in 4 cases. The toal effective rate is 86.7%. The total efficacy of the treatment group is better than the control group (P<0.01)ConclusionIn treating Lumbar disc herniation of acute stage, both the Warm Needling combined with Electroacupuncture and pure the Electroacupuncture are effective. However, Warm Needling combined with Electroacupuncture has obvious analgesic effect immediately, which is better than pure Electroacupuncture; For the acute lumbar disc prolapse patients, it has advantages in subjective symptom, clinical signs and living ability. It is safe and feasible, with higher clinical guiding significance. |
