| Objective:The purpose of this study was to understand the clinical characteristics of severe pneumonia and to contribute to individualized treatment for individuals with severe pneumonia.Method:Based on the medical record of cases we have collected from June 2012 to June 2014 at the first affiliated hospital of Guangxi Medical University, we divided the patients into three groups:youth, middle-age, and elderly group. We collected the following information:acceptance criteria, basic illness, ventilation therapy, etiology characteristics and initial antibiotic usage.Results:A total of 81 cases were examined (51 male and 30 female), with an average age of 63.51(SD=16.13). There were 8,30, and 43 cases in youth, middle age and elder groups. Of the 81 patients,40 cases were from SCAP and 32 cases were from SHAP. The difference in mortality rate is significant, with the mortality rate for two conditions being 42.9%(SCAP) and 78%(SHAP). Mortality differences between different age groups has no statistical significance. Additionally, compared with the patients in the middle-age groups, patients in the elderly group were more likely to suffer from high blood pressure,Cerebral infarction, coronary heart disease, arrhythmia, the difference is statistically significant (P<0.05). Among SCAP patients, higher probability of successful treatment using NIV was found in patients in the youth group. Among the three group, the mortality rate was highest among the elder group using tracheal intubation. Finally, the etiology of severe pneumonia mainly involves gram-negativebacterium, Acinetobacter baumanii, Pseudomonas aeruginosa showed more resistance to medicine. Frequently used antibiotics in our hospital were beta-1actam, fluoroquinolone.Conclusion:The mortality rate among all age groups are high, we need to pay attention to the treatment of severe pneumonia of all age groups. Elderly group has high proportion of basic diseases, the treatment of NIV in middle-aged group has high success rate of SCAP. The etiology of severe pneumonia mainly involves Gram-negativebacterium, and Acinetobacter baumannii and Pseudomonas aeruginosa resistant rate are high. Frequently used antibiotics in our hospital were beta-1actam, fluoroquinolone. |
