| Purpose:CI is common diseases of nervous system with features of high morbidity, ratio of cripple, mortality, recurrence and more complications. With the rapid development of medical treatments, the early detection of ischemic CI is obviously higher and the fatality rate decreases significantly. But movement, speech and cognitive dysfunction which caused by infarction are the main disabled performances. At present there is no specific clinical solution, and it affects patients’ quality of life seriously, increasing the burden of patient, family and society.As a major means of modern rehabilitation training, exercise therapy is widely used in the recovery of CI.Studies prove that exercise therapy, the main part of rehabilitation training is an important part of the multi-disciplinary comprehensive treatment, which is proved to be beneficial to the restruction of brain structure and function, activating the partial residual function, so as to promote and improve the patient’s cognitive and behavioral ability, movement and sensory function, reducing the degree of disability and the incidence of complications and improving the patient’s daily life activities and social adaptation ability.In recent years, some scholars have carried out the research that related treatment mechanism of exercise training on ischemic CI on multi-dimension. Part of the study discusses the sports training influence on neurocyte glucose metabolism of infarction areas of ischemic CI. But related report is limited, and failed to fully clarify the therapeutic mechanism of exercise training on ischemic CI.This experiment has established the ischemic CI model of rat, giving them intervention of exercise training, and observing the change of glucose metabolism index of the infarction area and its nervous behavior before and after intervention through methods of exercise test, balance beam test, screen test, micro PET, 18F-FDG and SUV to verify the treatment effect of exercise training on ischemic CI and explore its possible mechanism on motor nerve in recovery of ischemic CI.Methods:1.Objects and interventions: through the random number table method, 28 SD rats that from the animal laboratory center of Sichuan medical university were randomly divided into four groups, which are the shame operation group, the model group, the rehabilitation group and the drug group respectively(n=7).The MCAO model was made based on the method of suture-occluded for the rats from the model group, the rehabilitation group and the drug group. And only neck vascular exposure processing was done for the shame operation group.After the failure of rats molding and flake off, cover group was carried out by random principle during the experiment to guarantee the number of rats of all groups.The shame operation and model group were raised normally; rehabilitation group was given the training of balance beam, rotating stick and roller to train the abilities of grasping, balancing, walking and rotating of rats; and Edaravone intraperitoneal injection treatment was given to the drug group. 2.Observation index: ①Test evaluation of exercise, screen and the balance beam were carried out with four groups of rats in 1d, 7d,14 d, 21 d and 28 d five time respectively, observing the movement and neural function changes of rats.②Using micro PET/CT detect SUV values of FDG of four groups rats in 1d and 28 d times, observing the sugar metabolism changes of brain nerve cells in different intervention cases.3.Statistical processing: Using statistical software(SPSS17.0) to do statistical analysis for data, all the data were expressed by ±s, and the data was confirmed obeying the normal distribution.Test standard is α= 0.05, before and after the comparision of the treatment in the group, paired t test can be used, and one-way analysis of variance can be used in group comparison. And analyzing the the test score values of SUV, sport, screen and balance beam. p<0.05 has statistical significance and p<0.01 with a significant statistical meaning.Results:1.A rat MCAO model that made by suture-occluded method can simulate ischemic CI, neurobehavioral score is high, the infarction area and volume is stable, glucose metabolism of nerve cells in infarction area changes obviously.2.After the molding, the waking shame operation group rats have no abnormal neurobehavioral performance, and evaluation without exception; all MCAO model rats showed anomalies in different degrees in the test of sports, screen and the balance beam, and all scores of 1d are unusual, but each group is p>0.05 with no statistical significance. In the rehabilitation group the score of 7d, 14 d, 21 d and 28 d is p<0.05 or p<0.01 with statistical significance. For the drug group the score of 7d and 14 d is p<0.01 or p<0.05 with statistical significance, but the score after 28 d is p>0.05 with no statistical significance.3.The shame operation group rats found no brain infarcts in the 1d, 28 d and no abnormal change in glucose metabolism; the area and volume size of infarction area of MCAO model rats is different in 1d, glucose metabolism of nerve cells decreased in infarction area in a different degree, but the comparison between groups is p>0.05 with no statistical significance; the infarction area and volume in the rehabilitation and drug group decreased and narrowed in a different degree compared with the model group that after 28 d, and sugar metabolism of nerve cells in the infarction area increases; compared with model group rehabilitation group is p<0.01, however, compared with model group the drug group is p<0.05, has a statistical significance, but the rehabilitation group has a significant statistical significance.Conclusion:1.Suture-occluded method can successfully simulate the MCAO model of rat, the movement and neural function of MCAO model rats decline. 2. The deletion of motor neurologic function symptoms and the existence of CI rats are related with the reduce of sugar metabolism of nerve cells in the infarction area. 3. Rehabilitation training would help CI infarcts area reduces and volume shrinks, the sugar metabolism of infarction area increased. 4. Rehabilitation training can promote the recovery of movement and neurological function in CI rats,the mechanism may have a relation with that rehabilitation training can probably reduces CI infarction area, shrinks its volume and sugar metabolism of nerve cells increases in infarction area. |
