Object Patients with chronic heart failure(CHF) were separately measured plasma brain natriuretic peptide(BNP),.and heart failure echocardiography index(HFEI),which synthesizes the multinomial parameter of echocardiography. To investigate the relationship between the BNP, HFEI and CHF, and explore their combination evaluating cardiac patients with CHF the value of the function, and objective indicators for improving better diagnosis and clinical assessment of CHF.Methods To prospectively study 92 patients of CHF(as the CHF group) and 30 healthy people(as the control group).The CHF group includes male 58 and female 34 cases; NYHAⅠlevel 10, Ⅱlevel 18, Ⅲlevel 26, Ⅳlevel 38 cases; 44 cases of diastolic heart failure and 48 cases of systolic heart failure. The control group includes male 58 and female 34 cases. Recording all the research subjects the participants age, gender, height, weight and other basic information. Recording detection the blood routine, biochemical and BNP index of all the research subjects. Recording the index of the echocardiography: left ventricular end-diastolic diameter(LVDd) thickness of interventricular septum(IVS), left atrial diameter(LAD), left ventricular posterior wall thickness(LVPW), left ventricular EF value mitral valve early diastolic blood flow velocity(E) the largest late mitral diastolic blood flow velocity(A) the largest E peak flow deceleration time(DT) pulmonary vein systolic blood flow peak(S) of pulmonary vein(D) the pulmonary artery diastolic blood flow peak systolic valvular regurgitation in degree or narrow degree.Examined by the echocardiography to calculate the heart failure of ultrasonic index(according to the index of the echocardiography).The results were compared with the NYHA classification standard. The correlations between the BNP and the heart failure of echocardiography index, LVEF were analyzed separately.Results The BNP levels and the heart failure of echocardiography index in CHF group were higher than those in control group(P<0.01).In the different types of HF, there were significant differences between BNP(systolic heart failure 734.8±98.86 vs diastolic heart failure 387.8±50.60,P>0.05)and HFEI(systolic heart failure 4.12±0.96 vs diastolic heart failure 2.96±0.78, P<0.01). The correlation between BNP and the NYHA classification standard was remarkable positive(r=0.53, P<0.01), the correlation between BNP and LVEF was negative(r=-0.65, P<0.01), and the correlation between BNP and the heart failure of ultrasonic index was remarkable positive(r=0.76,P<0.01). Smoking and drinking can become risk factor for heart failure. Coronary heart disease(46.7%) and hypertension(32.6%) are major basic diseases incidence of heart failure.Conclusions BNP levels and the heart failure of ultrasonic index are both good markers used to diagnosis CHF and evaluations of the CHF. The correlation between BNP and the heart failure of ultrasonic index is remarkable positive. HFEI can be used to identify systolic heart failure and diastolic heart failure.
