BackgroundPrimary Liver Cancer(PLC),short for liver cancer,is the fifth cancer in the world,and new cases are more than 1 million people each year which seriously damage to human life and health. Primary Liver Cancer include Hepatocellular Carcinoma(HCC),intrahepatic bile duct Carcinoma, Hepatocellular Carcinoma and intrahepatic bile duct Carcinoma mixed type,The incidence of liver cancer.Due to liver cell Carcinoma accounts for roughly 85%-90% of primary liver cancer,so this paper referred to in “cancer” is mainly refers to HCC. HCC in the world is on the rise and its morbidity and mortality is respectly the fourth and second in China.According to the Global Cancer Report 2014,the new cases and death toll of liver cancer is the first in the world,and HCC deaths accounted for 51% of the world.Viral hepatitis,alcohol,hepatic cirrhosis,yellow aspegillus toxin,drinking polluted water,genetic factors,nitrosamines are possible causes.In China,chronic viral hepatitis in many pathogenic factors is the most main cause.The epidemiological found that about one-third of patients have a history of chronic hepatitis,85%-90% patients are related to Hepatitis B Virus(HBV) and 5%-8% paients with Hepatitis C Virus(HCV) antibody positive.Early HCC onset conceals,lack of typical symptoms, which mostly has entered a period.and the magligant degree is high,easy to recurrence and metastasis,low surgical resection rate,high recurrence rate,poor prognosis and early diagnosis is difficult so that delay the best trentment period.Therefor,early detection and early diagnosis is the premise to improve the effect of treatment.The diagnosis of liver cancer is mainly according to the serological detection,imaging examination and liver puncture,and serological detection is mainly Alpha fetoprotein(AFP) which is a useful serological marker for diagnosis HCC.However,low sensitivity limits its effection which urgently need for us to seek new biomarkers to early diagnosis of HCC.Talin which is an important part of adhension cell gelling spot center is a kind of macromolecular cytoskeletal protein and can be combined with a variety of cell adhension molecules. Talin which can regulate cell adhension,metastasis and dissemination is the firstly identified cytoplasmic protein partner of integrins.and promote the occurrence and development of tumor.Related studies in recently years show that Talin participate in tumor metastasis and invasion.Talin-1 is an important part of Talin family and play an important role in tumor adhension and migration. Talin-1 is mainly expressed in prostate gland,kidney,stomach,liver,spleen,lung,and vascular muscles and its overexpression can promote prostate cancer cell adhension, metastasis,and invasion.Rencently related researches identified that Talin-1 is a novel molecular marker for HCC progression.However,its clinical validation as a serum marker for diagnosis and prognosis of HCC patients has not been investigated. AimTo investigate serum Talin-1 level in HCC patients and to access the diagnostic value of serum Talin-1 compared to the most feasible traditional biomarker alphafetoprotein(AFP) for the diagnosis of HCC. MethodsTalin-1 was detected by Enzyme Linked Immunosorbent Assay(ELISA) in serum samples from 120 subjects including 40 cases with HCC, 40 caseswith liver cirrhosis(LC) and 40 healthy controls(HC). Statistical analysis:All the datas were analyzed by SPSS17.0 statistical package.The differences between classification variables and continuous variables are compared by Chi-square test,T-test or one-way analysis of variance(ANVOA).The comparision and correlation between Talin-1 and other clinical characteristics in three groups are analyzed using Kruskal-Walllis test(non-parametric test),Mann-Whitny U test(non-parametric student t test) and Spearman level correlation analysis.All the tests were two-tailed and the level of statistical significance difference was α=0.05.The difference was statistically significance(P<0.05) ResultsROC curve analysis was used to create a predictive model for TLN1 relative to AFP in HCC diagnosis. Serum levels of Talin-1 in hepatocellular carcinoma patients were significantly higher compared to the other groups(p=0.000 < 0.05). The diagnostic accuracy of Talin-1 was higher than that of AFP regarding sensitivity, specificityin diagnosis of HCC. ConclusionTalin-1 maybe a potential novel biomarker with a higher sensitivity and specificity compared to the traditional biomarker AFP for the early diagnosis of HCC.
