Objectives The purpose of this paper is to study the influence of Shexiang Wulongpilling on rat adjuvant arthritis(AA)synovial pathological morphology and hypoxiainducible factor-1α (HIF-1α) and matrix metalloproteinase-2(MMP-2) expression.Andexplore whetherthe Shexiang Wulong pilling in the treatment of rheumatoid arthritis (RA)is associated with the expression level of HIF-1α, MMP-2regulation.Methods The research objects are60healthy male Wistar rats and the rats are selectedrandomly.The normal control group includes12rats (group A) and the48of adjuvantarthritis rats model.The rats are randomly divided into control group (group B),low dose ofShexiang Wulong pill group (group C),shexiang Wulong pill in middle dose group(groupD) and high dose group(group E).Each group includes12Wistar rats and all the rats arekilled and tested after intragastric administration for2weeks.Shaving the skin cut footmetatarsophalangeal joint, to observe the histopathology of synovial with light microscope,and the method of MMP-2synovial membrane the cellular expression of HIF-1α,immunohistochemistry was used to detect.Results1Observation of synovial morphology under microscope: the synovial tissue ofnormal group is normal microscopically, the surface of synovial cell is smooth, neat, cellmorphology and number of normal, no infiltration of inflammatory cells and vascularproliferation. Fibrosis and papillary hyperplasia are not founded on Synovial tissue.Congestion swelling, synovial tissue is significantly in the model group. synovial cells isarranged irregular, active proliferation and cell layer significantly increased. The amountof inflammatory cell infiltration and lymphoid follicles are founded. Synovial capillarypannus increased, pannus surface is papillary or fence like processes. Articular cartilagesurface is not smooth and matte, tissue defect is obvious. The low dose group of ShexiangWulong pill significantly reduced the synovial tissue swelling hyperemia, synovial reducecellular level. Synovial hyperplasia, inflammatory cell infiltration and capillary formationis obvious improvement than the model group. shexiang Wulong pill in middle dose groupsynovial tissue swelling hyperemia, cell level to reduce the degree of low dose group arenot significantly improved, joint and articular cartilage is still mild defect. Shexiang Wulong pill in high dose group synovial tissue, swelling hyperemia, lymphocytesinfiltration and pannus formation are improved in comparison with the model group andlow dose group significantly.2The expression of HIF-1αand MMP-2by synovial cellimmunohistochemical method:the positive expression rate of HIF-1αand MMP-2of modelgroup are significantly higher than those in normal group, statistically significant (P <0.01). The expression of HIF-1αand MMP-2of Shexiang Wulong pill low and high dosegroup decreased significantly (P <0.01) than those in normal group.The decreased levelsof high dose group are significantly improved (P <0.05) than those in the model group andlow dose group.Conclusions Shexiang Wulong pill can reduce the expression level of HIF-1α and MMP-2.It could obviously reduce the degree of synovial lesions of AA rats ankle so as to controlthe formation of synovial cell in AA rats hyperplasia and pannus, this is one of thetreatment mechanism of RA.
