Safety And Efficacy Of Thulium Laser Enucleation Of The Prostate Versus Transurethral Resection Of Prostate For Treatment Of Benign Prostate Hyperplasia:a Meta-analysis

Objective To objectively evaluate the safety and efficacy of Thuliumlaser enucleation of the prostate(TmLEP) versus transurethral resection ofprostate(TURP) in treatment of benign prostate hyperplasia(BPH).Methods Such databases as PubMed, Embase, Cochrane Library,Wanfang, VIP, Wanfang, CNKI were searched by prospective casecontrolled trial(CCT), randomized control trial(RCT) andquasi-randomized control trial (Q-RCT) about TmLEP versus TURP for intreating BPH online. By inclusion and exclusion criteria for studyselection, assessment and data extraction conducted, and then Metaanalysis were adopted by the RevMan5.2software.Results Five studies were identified with544patients．The results ofMeta analysis showed that: a）safety indicator: comparing TURP, TmLEPhad less intraoperative bleeding[WMD-7.07,CI(-8.99,-5.16)P<0.00001],less decrease of serum sodium salt[WMD-3.07, 95%CI(-4.36,-1.78)P<0.00001],less blood transfusion percentage[RR0.10,CI(0.02,0.42) P=0.002],shorter catheterization time[WMD-0.92,95%CI(-1.11,-0.73)P<0.00001] and shorter hospital stay[WMD-1.82,95%CI(-3.36,-0.28)P=0.02]. But there was no significant differencein operation time and postoperative complications(P>0.05). b)efficacyindicators (IPSS, PVR, Qmax, QoL): Compared with the baseline,significant improvement in IPSS,QoL, Qmax and PVR were obviousfound before and after surgery in both groups. There were statisticaldifference in IPSS at12-month and in QoL at6-month between twogroups.Conclusions TmLEP is a safe and effective procedure for treatmentof benign prostatic hyperplasia. Compared with TURP,TmLEP was notonly associated with less intraoperative bleeding, less decrease of serumsodium salt, less blood transfusion and shorter catheterization time,butalso had similar therapeutic effects.
