The analysis of the causes of the revision hip arthroplasty ObjectiveThis retrospective study Collected the data of the inpatients who accepted the revision hip arthroplasty from2008to2013in the orthopedic department of the2nd Xiangya Hospital of Central South University, and analyze the causes and regulations of the revision hip arthroplasty.MethodsThis retrospective study Collected77inpatients who accepted the revision hip arthroplasty from2008to2013in the orthopedic department of the2nd Xiangya Hospital of Central South University. Discuss on the major cases and analysis the data of revision causes, time from primary hip arthroplasty to revision, preoperation results of CRP and ESR, germiculture, degrees of the cup abduction and anteversion.ResultsThe causes of revision hip arthroplasty include:53aseptic loosenings,12infections,7periprosthetic fractures,2dislocations and3accetabular wears. The interval time from primary hip arthroplasty to revision hip arthroplasty are11below1year,24between1and5years,22between6and10years,14between11and15years,4between16and20years,2surpass20years. The results of ESR are52.3±20.1mm/h in infection patients and20.5±9.4mm/h in aseptic loosening patients, there were significant difference between two groups (P<0.01). The results of CRP are36.05±33.77mg/L in infection patients and4.96±3.42mg/L in aseptic loosening patients, there were significant difference between two groups (P<0.01). The results of germiculture include5staphylococcus aureus,2staphylococcus epidermidis,1blastomyces albicans, and no result4. The average time of77implant survival were7.4years,6.8±4.4years for cement hip arthroplasty,6.3±4.8years for cementless hip arthroplasty,13.9±8.0years for hybrid hip arthroplasty. There were no significant difference between cement hip arthroplasty groups and cementless hip arthroplasty (P<0.01), but there were significant difference between those two groups and hybrid hip arthroplasty group (P<0.01)。In53aseptic loosenings patients, the average survival time of18patients implant with import prosthetics was9.6±4.7years, and5.9±5.2years for those35patients implant with domestic prosthetics, there were significant difference between two groups (P<0.05)。ConclusionThe number and proportion of revision hip arthroplasty rises up every year. Aseptic loosening is the most important cause, the average survival time of import implants was longer than that of domestic products. The proportion of infection is still large in China. A number of inpatient who need accept the revision hip arthroplasty results from the improper position of prosthetics because of poor technique. The hip arthroplasty permission rule is imperative.
