Background:With the widely application of glucocorticoid in clinic,its complications work up the attention of the medical workers,such as osteonecrosis of the femoral head,and have a growing trend.At present,the application of glucocorticoid has become the one of the most important reason for non-traumatic femoral" head necrosis, with the development of the disease, the patient will feel hip pain and restricted movement, claudication, lower limb shortening, and in the end, can cause collapse of the femoral head, and seriously affect the labor ability and quality of life of patients, patients must total hip replacement, and brings great economic burden to family and the society. At present, the exact mechanism of glucocorticoid leading to osteonecrosis of the femoral head is not very clear.In order to better prevention and treatment of the femoral head of glucocorticoid-induced necrosis, it is particularly important to research its pathogenesis.Objective:To observe the expression levels of bone morphogenetic protein-2(BMP-2)、 runt-related transcription factor-2(Runx-2)、osteoprotegerin (OPG)、peroxisome proliferators-activated receptorγ (PPARγ) and receptor activator of NF-κB ligand (RANKL) mRNA inside the femoral head of glucocorticoid-induced necrosis in advanced stage.Methods:The bone tissue of femoral head were collected from6patients suffering GNFH (experimental group) and from6patients suffering fracture of femoral neck (control group). There are4men and2women in the two groups respectively, aged25to76years (mean50.6years in the experimental group and mean51.4years in the control group). The patients of experimental group received over3weeks’glucocorticoid treatment or more than1week’s high-dose glucocorticoid therapy in recent2years, and patients of the control group did not receive hormone therapy. In2groups,the total RNA was extracted from the bone tissue of femoral head.At last, the expression level of BMP-2, Runx-2, OPG, PPARy and RANKL were measured with RT-qPCR.Results:The expression of BMP-2mRNA、Runx-2mRNA、OPGmRNA、PPARyγmRNA and RANKLmRNA was lower in the experimental group compared with the control group,with the2-ΔΔCt being0.38±0.05、0.37±0.05、0.44±0.06、0.47±0.05、0.43±0.05respectively. The differences observed were statistically significant (P<0.01).Conclusion:The necrotic zone of the femoral head of glucocorticoid-induced necrosis in advanced stage is full of necrotic bone tissue.So the expression of BMP-2mRNA、 Runx-2mRN、OPGmRNA PPARymRNA and RANKLmRNA was lower in the experimental group compared with the control group.
