| Melasma, as a common pigment disorder skin disease, since it affects appearance, brings great mental stress and psychological burden to the patients and have rather a large influence over the quality of life, although it have not life-threatening. The etiology and pathogenesis of Melasma are not fully educidated yet. The modern medicine has not an ideal treatment method yet. TCM syndrome differentiation and treatment of Melasma have a long history, and have its’own individual theory and the methods of treatment. The contemporary traditional Chinese physicians, following the principle of syndrome differentiation, after summing up the repeatedly practice, had accumulated rich clinical experience. However, there exist obvious differences in the dianogsis and treatment experience between different academic schools. In addition, the experiences of famous old expert, were lack of a systematic induction. And these factors limic the young doctors’heritage and development.ObjectiveAnalyzing the medical cases of Melasma of last years, to explore the potentially meaningful herb-symptom-symdrone rules, so as to help to grasp the right direecion of syndrome differentiation during the process of diagnosing disease and making prescription.MethodsSearching the CBM and finding out the medical cases of Melasma. Record the symptoms, syndrome differentiation of syndromes types and names of Chinese traditional medicines with the same standard specification, and then, use the SPSS Clementinel2.0software to mine the data. At last, using the theory of Chinese medicine to analyze the results.Results There were a total of99articles of Melasma contemporary medical cases including130cases enrolled.1. There were200kinds of traditional Chinese medicine used in the130cases, a total frequency of1736.27kinds of traditional Chinese medicine are used in the frequency of greater than15percent, they are Dang-Gui, Chuan-Xiong, Chai-Hu, Bai-Shao, ext. Among which, Danggui was used in a very high frequency. In130cases, there were112cases used Danggui, and it’s frenquency of use was86.15%. Among the high used frequency of Chinese herbs, there were10kinds of Chinese herbs with the function of promoting blood circulation for removing blood stasis, and their frenquency of use was37%among the high used frequency of Chinese herbs. And there were11kinds of Chinese herbs with the function of invigoration, and6kinds of herbs with the function of activating qi flowing. The bitter medicine, sweet medicine and pungent medicine is frequently used. And the warm medicine and the cold medicine is used in the same frequency.2. Collecting73symptoms, which total frequency is739times. Among which emotional disorders, such as irritability and insomnia dream, and menstrual disorders such as amounts little menses, dark-color menses, dysmenorrheal, breast pain, are the main associated symptoms. There were56tongue manifestation and pulse manifestation, a total frequency of428times, dark red tongue red tongue, pale tongue, thin white coating, stringy pulse is the common tongue manifestation and pulse manifestation.3. Analysis of syndrome frequency shows that there were41syndromes, a total frequency of229times. Among which, the syndrome of qi stagnation and blood stasis and the syndrome of stagnation of liver qi are the main syndromes of Melasma, especially the syndrome of qi stagnation and blood stasis which accounted for50.41%in121cases.4. Occording to the analysis of association rules of traditional Chinese medicine and traditional Chinese medicine,28couplet medicines were being mining out. And among these28couplet medicines, there were13couplet medicines are compose of two medicines that both with the functions of promoting blood circulation,9couplet medicines are compose of the medicines that with the functions of promoting blood circulation and promoting the circulation of qi,8couplet medicines are compose of the medicines that with the functions of promoting blood circulation and replenishing blood,3couplet medicines are compose of the medicines that with the functions of benefiting qi and promoting blood circulation. And the most support and the highest confidence couplet medicines are compose of the medicines that with the functions of promoting blood circulation for removing blood stasis, invigoration and promoting the circulation of qi.5. Mining out34results of the association rules of Chinese medicines and symptoms. It showed that the patients with the symptoms of irritability and breast pain can commonly use DangGui, ChaiHu and BaiShao.6. Mining out6results of syndromes and Chinese medicines. It showed that DangGui, ChuanXiong and TaoRen were good for syndrome of qi stagnation and blood stasis, and ChaiHu, DangGui, BaiShao were good for syndrome of stagnation of liver qi. Conclusion1. The high frequency of medicines showed that the traditional Chinese physician nowadays frequently use the benefiting qi and nourishing blood method, the regulating qi-flowing method and the regulating blood method.And the promoting blood circulation for removing blood stasis method was commonly used.2. The higher frequency of symptoms such as irritability and insomnia dream and symptoms of menstrual disorders can be attributed to syndrome differentiation of liver in syndrome differentiation of zang-fu viscera. It showed that the etiology of Melasma gone hand in hand with disorder of liver. So the treatment must combine with mental healing.3. Qi stagnation and blood stasis was the key pathogenesis of Melasma. Whether it was qi disease involving blood or blood disease involving qi, eventually caused Qi stagnation and blood stasis, and blood stasis in main and collateral channels can caused Melasma.4. Obtained28couplet medicines of accuracy, universality and high availability. They showed that in the threatment of Melasma, the method of promoting blood circulation for removing blood stasis, the method of benefiting qi and blood and the method of dispersing stagnated liver qi were inseparable. They were supplementary to each other.5. Mining out34rules of Chinese medicines and symptoms. DangGui can almost used in all symptoms that appearenced in Melasma. And ChuanXiong, ChaiHu, Baishao both could be widely used in clinical. The rules of Chinese medicines and symptoms showed that the TCM doctors in the treatment of chloasma focused on the methors of activating blood, nourishing blood and liver, and preventing disease. |
