Objective:To investigate the correlation between the levels of homocysteine(Hcy),folate, VitB12and the acute cerebral infarction (ACI), to explore the relationships between Hey and its nutritional factors, to study the association of MTHFRC677T polymorphism and Hey, folate and VitB12levels in the patients of ACI.To provide the clinical basis for prevention of ACI.Methods:From01.Dec.2012to01.Sep.2013, in the Neurology Department of Tianjin First Center Hospital,150patients with ACI (onset within3days) were collected as the ACI group, and175healthy person as control group. All cases were collected the reports of MRI/CT, the ill history, the life style and laboratory tests, plasma Hey detected by enzymatic cycling, folate and VitB12detected by immunofluorescence technique, lipid determinated by direct measurement, fibrinogen determinated by immune turbidimetric method. MTHFRC677T polymorphisms of ACI patients (n=48) were analyzed using PCR2RF-LP. Use IBM-SPSS19software to do statistical analysis(p<0.05).Results:1. Plasma Hey levels in ACI group (21.55±13.76) was higher significantly compared with control group (17.59±12.96)(p<0.05). Serum VitB12levels in ACI group (374.62±237.93) was lower significantly compared with control group (453.38±306.29)(p<0.05). There was no significant difference in two gruops (p>0.05)2. Significant relationship between Hcy and ACI was observed (R=0.12,P<0.05), Significant relationship between VitB12and ACI was also observed (R=-0.144,p <0.05),but no significant relationship between folate and ACI (p>0.05)3. Rate of ACI in mild Hcy elevated group (45.1%) and moderate Hcy elevated group (58.3%) was higher significantly compared with that of normal Hcy gruop(p<0.05).But no significant difference between the two Hcy elevated groups (p>0.05) was found.4. No simple linear regression relation between Hcy and folate, VitB12respectively (R2<0.3),even the levels of folate and VitB12in two Hcy elevated groups were significantly lower compared with that of normal Hcy group (p<0.05).5. The frequencies of C/C, C/T and T/T genotypes were35.43%,56.32%and8.25%respectively. The frequencies of T and C alleles were63.59%and36.41% respectively.6. Frequency of T/T genotype in moderate Hey elevated group (62.5%) was significantly higher compared with that in mild Hey elevated group (37%) and normal gruop (10%)(p<0.05)7. Plasma Hey level in T/T group (72.18±62.54)was higher compared with that of other groups significantly (p<0.05),while plasma Hey levels in T/C group (22.81±15.70) was no significant difference compared with that of C/C group (15.78±13.60)(p>0.05)8. There was no significant difference in the levels of plasma folate andVitB12among these three groups (p>0.05)Conclusion:1. HHcy is a risk factor for Acute Cerebral Infarction(ACI).2. VitB12may be a risk factor for ACI independenting of Hey.3. The linear regression relation between Hey and folat, VitB12is not definite.4. MTHFRC677T polymorphism is positively related with HHcy.5. The colleration between MTHFRC677T polymorphism and folate and VitB12is not clear or definite.
