Background and objective: Insulin resistance (IR) is closely correlated withcardiovascular diseases (CVDs). Retinol-binding protein4(RBP4) is a novel adipokine thatmay be a predictor of cardiovascular diseases. It is reported to be able to modulate theaction of insulin in different diseases. This study investigated the relationship between theserum RBP4and wаist circumf r ncе (W C), bоdy mаss ind x (ВMΙ), waist-hip ratiо(WНR), blood pressure, metabolic markers and insulin resistance index (HOMA-IR) innewly diagnosed essential hypertension.Methods From November2012to January2014,511subjects were enrolled accordingto gender and age-matched method in Southwest Hospital Medical Examination Center ofThird Military Medical University. These subjects were divided into newly diagnosedhypertensive group and normotensive group. There were297newly diagnosed hypertensivepatients (male201cases, female96cases), which were subdivided into two subgroupsobese hypertensive group (155cases, male105/female50) and non-obese hypertensivegroup (142cases, male96/female46); There were214cases of normal blood pressuregroup, which also were subdivided into two subgroups normal control group (131cases,male88/female43) and obese group (83cases, male58/female25). Serum RBP4levelsw r evaluated by nzym-link d immunоsоrb nt assay. Fasting serum insulin levels weremeasured by Radioimmunoassay. And the relationship between serum RBP4and BMI, WC,WHR, blood pressure, metabolic parameters and HOMA-IR was analyzed in the presentstudy.Results The BMI, WC, WHR, systolic blood pressure (SBP), diastolic blood pressure(DBP), uric acid (UA), total cholesterol (TC), triglycerides (TG), fasting blood glucose(FBG), fasting insulin (FINS), drinking rate, RBP4and HOMA-IR of hypertension groupwere significantly higher than that in normotensive group (P<0.05).Subgroup analyzes: The BMI, WC, WHR, SBP, UA, TG, FBG, FINS, drinking rate and HOMA-IR of obese hypertension group were significantly higher than that innon-obese hypertension group. However, HDL-C was significantly lower (P <0.05). SerumRBP4levels of obese hypertension group and non-obese hypertension group were similar(P>0.05). The indexes of ob s group compare with the control group: The BMI, WC,WHR, SBP, DBP, UA, TG, LDL-C, FBG, FINS, HOMA-IR of w r significаntly ig r, ut НDL-C wаs lоw r (P <0.05). Serum RBP4levels of obese group and control groupwere also similar (P>0.05).Cоrr latiоn analysis rev l d that, th re were significаntly pоsitiv ly cоrr latiоnbetween serum RBP4and WC, SBP, DBP, TG and alcohol consumption (r values were0.103,0.389,0.344,0.120,0.161, respectively, all P <0.05). After adjust for BMI, WC andWHR, serum RBP4remain was significantly positively correlated with SBP, DBP, TG andalcohol consumption (r values were0.385,0.334,0.096,0.123, respectively, all P <0.05).Multiple linear regression analysis showed that SBP and alcohol consumption wereindependent contributors for serum RBP4levels (P <0.05).Conclusions Serum RBP4levels were significantly increased in hypertensive patients,and positively correlated with SBP and DBP. However, there was no significant differencefor serum RBP4levels between obese and non-obese subjects. And the serum RBP4wasnot correlated with insulin resistance.
