Objective:To explore the clinical epidemiology feature of chronic spontaneous urticaria (CSU) and quality of life of CSU patients aiming to provide reference data for clinical comprehensive intervention and disease self-management.Methods:A total of362cases with chronic spontaneous urticaria coming to our hospital from March1st2012to December31st2013were collected as case group. A self-made questionnaire was used to collect information including prevalence, predisposing factors, treatment and so on. A total of100health cases coming for health check at the same period were selected as control group. The quality of life of both groups patients were evaluated by means of WHO Quality of Life Scale Brief(WHOQOL-BREF). Epidata3.1was used for data entry, and SPSS16.0for windows was used for statistical analysis.Results:(1) The CSU patients were aged11years to74years with mean of (37.5±13.4)years. A higher proportion was found from20to30years old. The male/female ratio is1:1.03.(2) The duration of CSU was6weeks to30years, with a median duration of6months. No seasonal trend was found and slightly higher proportion in the evening than other times.(3) Dyspnea was the most commen concurrent symptoms (20.7%) followed by abdominal and joint pain. About99.4%patients had varying degrees of itching, and98.3%had wheals with durations of shorter than12h.(4)92.0%of the patients had been taking antihistamines, and more than80%got effect.(5) The positive rate of skin prick test, serum sIgE and sIgG were65.7%,67.9%and32.1%, respectively.(6) Top three of the positive skin prick test allergens are house dust mites, mold and pollen. Food intolerances of sIgG positive patients, were among the top Milk/Milk Powder, egg/egg yolk, wheat/barley/oats/buck wheat/rye and tomatoes/carrots. Top five categories of food intolerance of sIgE positive patients were former crabs, mites combination, shrimp/prawn, cockroaches and freshwater fish combinations.(7)27.3%of the patients had a family history of allergies,67.1%have food allergies, and6.4%had a history of drug allergy.(8)13.5%of patients suffering from allergic rhinitis, and1.7%had asthma.(9) The quality of life and personal health in control group by self-evaluation were significantly better than the case group (Z=-8.785, P<0.001; t=-3.485, P<0.001).(10) The field of social relations got highest score (13.9±2.6), followed by physiological (13.4±2.1), psychological areas (13.2±2.0), and environmental field (13.1±2.2) in case group. The control group got significantly higher scores in psychological and physical domains than the case group (P<0.05).Conclusions:(1) Almost any age could suffer chronic spontaneous urticaria(CSU), with a high proportion aged from23to32years old.Different duration of CSU of each csae and no seasonal trend was found.(2) Chronic spontaneous urticaria had complex etiology and close correlation with serum sIgE, sIgG and allergen.(3) The majority of patients taking antihistamines effectively, and some patients had family history of allergies, food allergies and drug allergies.(4) Chronic spontaneous urtic-aria had negative influence on life satisfaction, health satisfaction and quality of life.
