Research On The Status And Countermeasures Of General Practitioners’ Standardized Training In Fujian Province

General practitioners’ standardized training (hereinafter, the GP’s standardizedtraining) is the key link to the construction of primary health care. Establishing asound system, improving the training mode and regulating the personnel of generalpractitioners are problems urgent to be solved during the process of medical andhealth system reform in our country. This research aims to investigate the status andcountermeasures of the GP’s standardized training in Fujian province, adoptingmethods including literature, questionnaires and interviews.The results showed asfollowed:(1)71.57%of the GPs believed that the work of standardized training was goodas a whole. Evaluation on the application of standardized training showed that morethan80%(the highest was86.28%and the lowest was81.37%) of the GPs believedthat standardized training was beneficial to the application of theoretical knowledgeand skills. Besides, evaluation on the reaction of standardized training showed morethan60%of the GPs (the highest was75.49%and the lowest was62.75%) appraisedit was good.(2)More than88.44%of the GPs had a certain understanding on thetraining-related policies, and were agree to the contents of the provisions. Because ofneeding to adapt to jod demand, to build self-capacity or dispatched by leaders,84.92%of the GPs were willing to participate in the standardized training. But the rest15.08%said it did not matter or showed unwillingness. Probably because most ofthem were above30years old, working in government institutions and had been giventhe mid-level or high-level professional title. Their job and family were relativelysteady, so they showed less willingness to participate in the standardized training.(3) This reaserch also focused on analyzing the situation of mastering GPs’knowledge and skills. Contrast analysis showed that the group of GPs whoparticipated in the standardized training had a better mastery of knowledge and skills than the group of GPs who didn’t participate in the standardized training. Analysis ofmultiple linear stepwise regression analysis showed that whether participating in thestandardized training or not was one of the factors which might influence on GPs’mastery of knowledge and skills (t=3.904,P<0.001). Participating in the standardizedtraining could improve the mastery of knowledge and skills.Meanwhile, there were still some aspects which should be strengthened andperfected on GP’s standardized training in Fujian. For example, the pertinence oftraining contents was not specific, and the arrangements of training were not flexibleenough.The teachers and the teaching materials were not standardized, and criteria ofevaluation were not clear enough. The attention of relevant medical institutions wasnot high, and the personnel management system was not reasonable. The constructionof training bases was immature and there was lack of communication among trainingbases. What’s more, the satisfaction of GPs was low (only47.74%), which greatlyimpacted the motivation of GPs’ work, et al.In order to solve the problems above, we has put forward the following fivesuggestions, hoping to provide a useful reference to the development of Fujiancommunity health services:(1) Improve the construction of the policies and arrangethe teaching content scientifically.(2) Establish an excellent teaching group andstrengthen the criteria of evaluation.(3) Put a high value on the position of GPs andinnovate the personnel management system.(4) Strengthen standardized constructionof the training bases and facilitate communication with each other.(5) Reform andinnovate the motivation system to promote the development of the GPs’ team.
