Objective:Oral lichen planus (OLP) is a common disease of oral mucosal. Lesions were manifestedin striated and patchy white damage,erosion and atrophy. The main pathological features arethe liquefaction degeneration of basal cells and inflammatory cell infiltration of laminapropria. Although the cause of OLP is not clear, but at present, many believe that OLP maybe a T cell-mediated immunoreactivity disease.Th17cells are the helper T cell subsets found in recent years. They play an importantrole in host defense of bacterial infection,mediating chronic inflammation,autoimmunediseases and cancer incidence. The RORγt is the specific transcription factor of Th17cells,that controls the differentiation and development of Th17cells.The costimulatory signal which is mediated by the costimulatory molecular4-1BB/4-1BBL provides the second signal of T cell activation. It can induce T cell activation,proliferation and anti-apoptotic,maintain long-term survival of T cells and improve cytokinesecretion. The interactions of costimulatory molecule receptor-ligand on the surface of T cellsand APC are critical to T cell activation and to promote T cell-mediated immune response.And this bi-directional signal control mechanism is significance to prevent excessive T cellactivation and to maintain a moderate.This project applies to detect the expression of RORγt and4-1BB/4-1BBL mRNA of orallichen planus patients and healthy peripheral blood with real-time fluorescent quantitativeRT-PCR.Possible mechanism of RORγt and costimulatory molecular4-1BB/4-1BBL involvedin the pathogenicity of oral lichen planus is preliminary explored on transcriptional level. And the possible theoretical basis for the pathogenesis of oral lichen planus is further provided.Methods:30samples of OLP patients peripheral blood came from OLP patients of the initialtreatment in Ningxia Medical University Hospital Dental Clinic.They were confirmed bypathology and excluded in combination with other autoimmune diseases,who did not receiveany hormones and immunosuppressive therapy.30cases of healthy human peripheral bloodsamples were from voluntary donations. They were no systemic and oral mucosal diseasediagnosis,whose liver and kidney function is normal. The study detected collected theexpression levels of RORγt mRNA and4-1BB/4-1BBL mRNA of peripheral blood from thepatients and the healthy human,using SYBR Green I real-time fluorescence quantitativeRT-PCR technology. All data were analyzed using SPSS13.0statistical software and the t-testbetween the two groups. P <0.05was considered statistically significant. Pearson correlationanalysis was used.Results:1.In the group of patients with oral lichen planus,the expression of RORγt and4-1BB/4-1BBLmRNA was higher than the healthy group. They are significant difference withstatistical analysis (P<0.05). After the conversion of the2-ΔΔCT,the expression levels of threekinds of mRNA in oral lichen planus group were3.087,3.320and4.005times of the healthygroup.2. In patients with oral lichen planus,the expression level of RORγt mRNA and4-1BBmRNA had no significant correlation by the statistical Pearson correlation analysis (P=0.073>0.05).3. In patients with oral lichen planus,the results of the statistical Pearson correlationanalysis in the expression levels of4-1BB and4-1BBL mRNA show both a positivecorrelation(r=0.4550, p=0.0115<0.05).Conclusions: 1. RORγt mRNA was highly expressed in the peripheral blood of patients with orallichen planus, indicating that the number of Th17cells may increase and immune injuryfactors increased. This indicated that Th17/Treg cell imbalance existed in vivo of OLPpatients,the destruction of the balance between Th17cells mediated inflammatory responseand Treg cell mediated immune tolerance was an important factor in the disease mechanism.2. The expression levels of costimulatory molecule4-1BB and4-1BBLmRNA in theperipheral blood of patients with oral lichen planus were abnormally high,indicating that4-1BB and4-1BBL costimulatory molecules may be involved in the onset of OLP andblocking the costimulatory pathway was expected to provide new ideas for treatment of OLP,while OLP patients may exist CD4+T cell activation, proliferation and abnormaldifferentiation.3. In patients with oral lichen planus,the expression levels of4-1BB and4-1BBL mRNAshow both a positive correlation,indicating that the costimulatory signal4-1BB/4-1BBL were abnormal actived and the expression were both increased when the body wasstimulated by external immune stimulation.All of this suggested that the elevated expressionlevel of costimulatory moleculesto may play a certain role in the pathogenesis of OLP.
