Research On Wuhan Health Record In The Application Of Community Health Service Centers And Influencing Factors

[Objective]Health record is a kind of information about residents’ health files, is oneof community health service center development health service bases, CommunityHealth staff as its main users and managers, on the health record create, manage, anduse affect the construction and development of health record. Through a survey ofcommunity health staff, understanding of current community health record: status ofmedical personnel to the residents as well as the subjective evaluation, analysisrestricting the health record creation, management and use of the influencing factorsand put forward reasonable proposals and remedies, by improving the managementand use process of health record, enabling the function of resident health record to bebrought into full play.[Methods]This research selected four community health service centers formQiaokou, Wuchang,Jiangxia,Jianghan district in Wuhan. Take the community medicalstaff as the investigation object, uses the self-made questionnaire to carry on survey,altogether provides questionnaires300, recycling effective questionnaire286,questionnaire recycling effectiveness95.3%. The quantitative analysis uses the SPSS18.0software to analyze data, including descriptive statistics analysis and chi-squareteat, Anova and the multiple-linear regression analysis of influence factor,the standardof teat is two-sided α=0.05.[Results]①Frequency of use health record, often used in the day-to-day healthservices accounted for only29.7%, Rarely used about37.4%. Community healthservice centers in different occupational medical personnel in the use of health record,with statistical significance,X2=18.305，P=0.006＜0.05.②health record mainly use in three aspects of and follow-up intervention of community health activities in the community with chronic diseases, health educationof the community residents as well as health information exchange, respectivelyaccounts for27.6%,21.5%and20.4%,but use of health record are few in clinicaldiagnosing and medical service, respectively accounts for10%and4.4%.③Different degrees in the use of health record computer system operating level witha difference, Fisher’s Exact Test Value=24.026P=0.016, the use of health recordsystems it is difficult8.7%, will use a number of71%. Health Record training staff of195people, accounting for68%,32%trained.52.1%of the people who most want toaccept forms of training through specific training and seminars to further understandthe health record.④Community Health staff on the evaluation of the role of the health record storageof health record, based on evaluation of the quality of community health and diseaseprevention in the first three, score（sx±S）are3.95±0.913、3.75±0.706and3.65±0.733.Community Health staff different titles in different role in the health record score,F=3.194，P=0.024<0.05.⑤Multiple-linear regression influencing factors of community medical staff on thehealth record to use as well as cognitive evaluation results of the analysis: occupation,wages and training are the main factors that affect the use and subjectiveunderstanding, theregression equation isY=4.254-0.115X6-0.287X41-0.270X42-0.299X43-0.196X9.The influencing factors ofhealth staff evaluation the health of record construction, government investment, theresidents do not coordinate as well as the lack of professional informationmanagement scores in the first three.[Conclusions]①Advantages of health record. Rational use of health record, dynamicupdates, and scientific management,we can understand the health status of communityresidents and the occurrence of diseases,that targeted community health preventionand clinical services.②The disadvantage of health record. The lack of governmentfunding; community lack of medical staff, health record low level of awareness; a narrow range of applications, the lack of training efforts; health record informationsharing, lack of health information professionals.③the opportunity of health recorddevelopment. Support of national policy as well as health information technologysupport provides a better environment for the development of healthrecord.④Challenge of health record construction. How to protect health record thesecurity in resident personal information, prevents question of law that theinformation revelation causes is one of threats; The information standardization issueis also another main threat of health record development.
