Objective:To investigate the value of detecting umbilical and spiral artery hemodynamic indexes by color doppler ultrasound in late high risk pregnant fetuses and the value of detecting UA and SA at the same time to predicting fetal prognosis.Methods:Clinical data of160singleton late pregnancies during antenatal care in the Shanxi Province Health care hospital for women and children from May2013to February2014were enrolled in our study.80cases of high risk pregnancies whose age were24to42years old and gestational age37to42weeks were seted as groupl,80cases of normal pregnancies whose age were25to34years old and gestational age37to41weeks were seted as group2,First routine obstetric ultrasound examination was carried out to exclude fetal abnormalities,determine fetal position, measure BPD,FL,HC,AC,HR, AFI,Color Doppler ultrasoud was used to examine the following blood flow paramete of fetal umbilical artery(UA) and spiral artery(SA) in high risk pregnancy group(80cages)and normal pregnancy group(40cases): systolic/diastolic ratio(S/D).resistance index(RI)and pulsatility index(PI) of UA and SA.The hemodynamic indexes and the perinatal outcome was analyzed respectively.The data were dealt with SPSS16.0, and were described in (x±s).P values<0.05was considered to be statistically significant. P values<0.05was considered to be statistically significant. The way t-test and χ2-test(Chi-square test) were used for statistical analysis.Results:①Compared with normal pregnant group,the S/D,RI and PI of UA and SA in high risk pregnant fetuses increased significantly-while,have obviously difference (P<0.05);②The adverse perinatal outcome incidence in the high risk pregnant group was23.75%,which was significant higher than0.38%of normal pregnant group,The adverse perinatal outcome incidences in the high risk pregnant group were significant higher than that of normal pregnant group,have obviously difference (P<0.05);③There were15cases when the S/D of UA and SA were unnomal at the same time, the incidences of poor perinatal outcome was80.00%,which was significantly higher than those from the group of U A and S A was single unnomal,and each group comparison showed significant differences (P<0.05)Conclusion:Use color Doppler ultrasoud to examine the following blood flow paramete of fetal umbilical artery(UA) and spiral artery(SA) in late pregnancies are with clinical value in making perinatal prognosis. Detecting UA and SA at the same time was valuable for predicting fetal intrauterine hypoxia and adverse perinatal outcome of high risk pregnant fetuses, Detecting SA is an additional method of UA to predicting adverse perinatal outcome.
