Background Hypertension in pregnancy,as a special disease in pregnancy,has been the focus of obstetric research and attention due to unclear pathogenesis and poor pregnancy outcome.This paper mainly studies the comprehensive monitoring of uterine artery,fetal umbilical artery and middle cerebral artery by color Doppler ultrasound instrument,which has practical clinical application value for evaluating the pregnancy outcome of HDP,guiding the clinical practice of timely treatment and timely selection of the timing of termination of pregnancy.ObjectiveAnalysis of the uterine artery Resistance index(RI)and Pulsatility index(PI)values of pregnant women with hypertensive disorders of pregnancy(HDP)and pregnancy outcomes,as well as Umbilical artery(UA)and Middle cerebral artery(MCA),resistance index,pulsation index,and the ratio of Peak systolic velocity/End diastolic velocity(S/D)to the Peak systolic velocity(S/D)were performed,to understand the application of ultrasound Doppler multiparameter analysis in HDP pregnant women in late pregnancy.MethodsEighty pregnant women with HDP at the age of 28-37 weeks were selected as the study subjects.According to the measurement standard of Color Doppler flow imaging(CDFI),RI,PI and S/D values of left and right uterine arteries,fetal umbilical artery and middle cerebral artery in late pregnancy were obtained.According to the outcome of pregnancy,the children were divided into low birth weight group and very low birth weight group.Asphyxia group and non-asphyxia group;Lung mature group and immature group.Results1.Birth weight in the perinatals found: in the comparison of very low birth weight group of umbilical artery RI,PI,S/D and pregnant women left uterine artery RI,PI,lower birth weight increased significantly,the difference was statistically significant(P< 0.05),middle cerebral artery RI,PI,S/D and right uterine artery RI,PI,change has no obvious differences between the two groups had no statistical significance(P > 0.05);2.In the study of neonatal asphyxia,it was found that in the asphyxia group,the blood flow parameters of the umbilical artery and the left uterine artery changed significantly,which was significantly higher than that of the non-asphyxia group(P<0.05),but the parameters of the middle cerebral artery and the right uterine artery did not change significantly,and the difference was not statistically significant(P>0.05).3.In the study on the correlation between pregnancy outcomes and lung maturation,it was found that compared with the lung maturation group,RI and PI of the left uterine artery were significantly increased in the immature group,and the difference was statistically significant(P<0.05).The indexes of the umbilical artery were increased compared with the mature group,and the difference was close to statistical significance(P>0.05).There was no significant difference in the frequency spectrum of the middle cerebral artery and the right uterine artery between the two groups(P>0.05).ConclusionThe results of this study show that uterine artery and fetal umbilical artery are important indicators for monitoring hypertensive diseases during pregnancy.Single monitoring of the middle cerebral artery to assess whether the fetus has intrauterine hypoxia has no practical clinical significance.The comprehensive monitoring and analysis of multiple parameters including uterine artery,fetal umbilical artery and middle cerebral artery can objectively evaluate the intrauterine safety of the fetus with preeclampsia,and have certain clinical application value for the evaluation of adverse outcome and guiding treatment of perinatal infants. |
