Objective:Study to compare and evaluate discuss the CTA and3D-TOF-MRA in dingnosis of intracranial artery system disease in clinical application value.Methods:In this study, patients between March2011to December2013to the hospital for treatment of86cases of intracranial lesions were retrospectively studied the results of a retrospective grouping of intracranial aneurysms is56cases,30patients with intracranial arterial stenosis patients. Carried CTA,3D-TOF-MRA and DSA all patients. Diagnostic criteria to DSA, to investigate the clinical application value of CTA and3D-TOF-MRA in intracranial aneurysms and intracranial arterial stenosis. And to calculate specificity, sensitivity, positive predictive value, negative predictive value.Results:CTA test results showed that the specificity, sensitivity, positive predictive value, negative predictive value of aneurysm patients can reach96.7%.83.9%.90%,80%. The specificity, sensitivity, positive predictive value, negative predictive value of arterial stenosis patients can reach93.3%、90%、93.3%、83.4%.3D-TOF-MRA test results showed that the specificity, sensitivity, positive predictive value, negative predictive value of aneurysm patients can reach70%、51.8%、80%、50.8%.The specificity, sensitivity, positive predictive value, negative predictive value of arterial stenosis patients can reach83.3%、80%、80%、73.3%.CTA test results showed that the sensitivity, specificity, positive predictive value, negative predictive value of the aneurysm patients is no not significant difference (p>0.05).3D-TOF-MRA test results showed that the sensitivity, specificity, positive predictive value, negative predictive value of the aneurysm patients is significant difference (p<0.05). CTA test results showed that the sensitivity, specificity, positive predictive value, negative predictive value of the arterial stenosis patients is not significant difference (p>0.05).3D-TOF-MRA test results showed that the sensitivity, specificity, positive predictive value, negative predictive value of the arterial stenosis patients is not significant difference (p <0.05).Patients with aneurysms group and arterial stenosis group testing result of CTA sensitivity, specificity, positive predictive value, negative predictive value compared with3D-TOF-MRA detection results is significant difference(p<0.05).Conclusions:CTA in the diagnosis of intracranial aneurysms with very high sensitivity and specificity. So CTA can be used as the first choice for the intracranial aneurysms check.CTA in the diagnosis of intracranial arterial stenosis with higher specificity and sensitivity. There was no significant difference with DSA in diagnosis of intracranial arterial stenosis. In the terms of sensitivity and specificity,3D-TOF-MRA is a little high in diagnosis of intracranial arterial stenosis.But it is low in diagnosis of intracranial aneurysms. So it can serve as a screening test method for intracranial arterial stenosis. As the radiation dose in the CTA examination and injection of contrast medium adverse reaction and other factors limited, CTA is in the inferior position of3D-TOF-MRA.3D-TOF-MRA without injecting contrast medium and radiation is effected by the blood flow velocity and vortex artifact.
