Objective:1.To explore the clinical value of real-time color Doppler ultrasound localization in intracranial hematoma surger.2.The changes of intracranial arterial blood flow parameters and blood flow patterns were observed on the basis of intracranial arterial pressure measurement by color Doppler ultrasonography.The intracranial pressure and intracranial arterial blood flow parameters And provide new guidance for surgical decompression and prognosis evaluation.Method:Randomly selected from March 2015 to October 2015 in our hospital line of 40 patients with intracranial hematoma surgery,of which 29 males and 11 females.Exclude patients with severe cardiovascular disease and other important organ damage.Intraoperative intracranial arterial blood flow patterns were observed by color Doppler ultrasonography.The morphological changes of intracranial arterial blood flow and the change of audio signal were observed.The intracranial arterial blood flow Vs,Vd,Vm,RI,PI index of the internal arteries were monitored dynamically after operation.The hematoma evacuation and the incidence of postoperative complications were observed.Compared with the cases of intracranial hematoma surgery,To evaluate the clinical effect of real-time color Doppler ultrasound and intracranial arterial blood flow analysis in intracranial hematoma surgery.Results:1.40 cases underwent successful operation under color Doppler ultrasonography.The detection rate and accuracy rate of the lesion were 100.0% in the operation.The monitoring result showed that the hematoma boundary was clear,and the head was reviewed every 12 hours.CT,hematoma boundary gradually blurred;2 cases of hematoma showed a low echo,and observed around the hematoma showed signs of edema.72 h after review of head CT,hematoma clearance rate was 95.0%.Intracranial arterial blood flow patterns showed a regular pattern with increasing intracranial pressure.In the early stage of intracranial pressure,intracranial arteries showed a high resistance spectrum,the blood flow spectrum shape is relatively complete,high systolic systolic and diastolic low flat;when intracranial pressure is higher than diastolic blood pressure showed diastolic Reverse and systolic positive blood flow;when intracranial pressure is higher than systolic blood pressure,the diastolic blood flow signal disappears,showing a sharp systolic "dagger"-shaped waveform.Intracranial pressure Vs and intracranial pressure was no significant correlation,Vd decreased with increased intracranial pressure,and intracranial pressure was negatively correlated with RI and PI increased with increased intracranial pressure,and intracranial pressure There was a significant positive correlation.Conclusion:1.TCCS can be clear in patients with hematoma location,size,etc.,to dynamic,real-time,continuous monitoring of intracranial hematoma morphology and structural changes,for intracranial hematoma surgery to provide accurate,Effective needle path,in clinical has certain guiding significance.2.TCCS can be in the intracranial arterial blood flow parameters and blood flow patterns of dynamic changes in the implementation of real-time,continuous monitoring,based on indirect reflect different levels of intracranial pressure increased the specific situation,Surgical decompression and evaluation of prognosis of patients with new guidance.
