Clinical Analysis Of117Cases Of Spontaneous Preterm Bitrh

Objective: To detect the different causes of spontaneous preterm birthgestational age,spuc time,the prognosis of premature infants and differentdelivery way influence on the prognosis of premature infants.Methods: A retrospective analysis from2009to2012in our hospital117cases of spontaneous preterm birth,comparative analysis of influence ofdifferent gestational weeks,different causes,different time and mode of deliveryon neonatal outcome.Results:①The most important cause of leading to spontaneous pretermbirth is intrauterine infection,about34.19%of the total number ofresearch;Secondly to scar the uterus,twin pregnancy,which accounted for17.95%of the total;Followed by abnormal fetal position (mainly refers to thebreech),accounting for11.11%of the total,polyhydramnios accounted for9.40%of the total,reproductive tract abnormalities accounted for8.55%of thetotal.The breech32weeks ago the larger proportion,about24.24%;It decreasedafter32weeks,about5.95%.While the intrauterine infection,uterine scar andtwin pregnancy were no gestational age difference before and after32weeks ofgestation,were main causes of premature delivery.②Compared to other causes and risk factors of prematuredelivery,intrauterine infection keep pregnancy for most short time,and theexistence of intrauterine infection even be anti-infection treated,the pretermbirth is often inevitable.③Before32weeks,the cesarean delivery can reduce the incidence ofneonatal asphyxia and intracranial hemorrhage（P＜0.05）;after32weeks,cesarean section can only reduce the incidence of intracranial hemorrhage（P＜ 0.05）,but in terms of neonatal asphyxia in cesarean section with no significantdifference between the normal delivery（P＞0.05）.④Premature rupture of membranes before term premature birth isspontaneous preterm neonatal infection rate increased obviously（P＜0.05）.Conclusions:①Intrauterine infection is the main cause of spontaneouspreterm birth,because of intrauterine infection aura of premature spuc shortesttime.②Based on the full consideration of the possibility of survival ofpremature infants, cesarean can significantly reduce the incidence of neonatalasphyxia and intracranial hemorrhage.③Compared with spontaneous pretermbirth,the neonatal infection rate was higher in premature rupture ofmembranes.④Prenatal active prevention of infection can effectively reduce theincidence of premature birth,reduce perinatal mortality and morbidity.
