Objectives. This study was designed for patients after coronary stent implantationwith Transitional Care Model(TCM). The purpose was to improve patients’ treatmentcompliance,the quality of life, reduce readmission rate, improve the clinical efficacy of thedisease, reducing the patients’ family financial burden through this method.Methods.1.Phenomenology method was used. Sixteen patients after coronarystent implantation in the hospitals of Suzhou were invited to participate in asemi-structured interview individually, and used Colaizzi method to analysis the data.Transitional care intervention was based on the results.2. Implementation of transitionalcare model. This study was a randomized controlled and single-blind study. Patientsafter coronary stent implantation were recruited by convenience in cardiovascular wards inthe hospital of Suzhou. They were spilted into two groups based on the randomized table.The experimental group received the TCM-based intervention while the control groupreceived the conventional intervention conducted by nurses of the ward. There were twoparts of the intervention given to the subjects in the experimental group. The subjectsreceived the face-to-face programmes and the reading materials based on their readiness ofchange when they were hospitalized and they received the telephone and home visitingintervention after they discharged. The change of treatment compliance, the quality oflife, readmission rate and other clinical outcomes were compared between two groupsfrom two weeks to one month and three months later after their being discharged.Results. There were four themes: worry the obstacles of transition to family; expectto return to home, get care and support of the community; lack of knowledge ofrehabilitation disease; desire to achieve self-worth. A total of146subjects were recruitedinto this study,6subjects dropped out of this study. Results were presented on theremaining140subjects with70in the experimental group and70in the control group.Results revealed that patient’s treatment compliance was higher (P<0.05) in theexperimental group than in the control group, and especially in control three high, smoking and exercise dimension(P<0.01). The quality of life was higher(P<0.01) in theexperimental group than in the control group, there were significantly changes(P<0.01) insymptoms, psychosocial, adverse reactions and treatment satisfaction dimension. Theblood pressure was lower at the experimental group (P<0.05at two weeks and one monthdischarged, and P<0.01at three month). The systolic blood pressure lowered6.57mmHgand the total cholesterol lowered0.65mmol/L at three month after discharged. The BMIlowered0.66kg/m2at three month after discharged. The readmission rate waslower(P<0.01) at the experimental group than in the control group.Conclusions. The TCM-based intervention to patients after coronary stentimplantation can effectively improve patients’ treatment compliance,the quality oflife,reduce readmission rate. The findings provide means for the nurse to find aneffective way to help patients after coronary stent implantation to transit to work and life.
