Objective:Based on the research on the distribution regularities of the TCM constitutions types and syndromes of chloasma patients, analyze the dependency between the TCM constitutions types and syndromes, discuss on the influence of the constitution on the susceptibility and clinical syndrome type of chloasma and study on the role of the research on the care of the constitution in the prevention and cure of chloasma.Methods:Collect126patients and exclude6among them. A total of120patients have completed the whole research and have been incorporated in the analysis. The diagnostic criteria of diseases follow the diagnostic criteria for chloasma revised in2003by the pigment team of the Special Committee for Skin Disease of the Chinese Association of the Integration of Traditional and Western Medicine; and the diagnostic criteria of constitutions comply with the Constitution in Chinese Medicine Questionnaire prepared by the "Study on the Categories ofConstitution in Chinese Medicine" of the "211" project of the10th Five-Year Plan of the Ministry of Education. The TCM constitutions can be divided into nine basic categories, Yin-Yang harmony, Qi deficiency, Yang deficiency, Yin deficiency, phlegm-dampness, damp heat, blood stasis, Qi depression and special constitution. The diagnostic criteria of syndromes follow the ZYYXH/T352-2012identification of blackish facial patch in the Diagnosis and Treatment of Common Skin Diseases in Chinese Medicine released in2012by the China Association of Traditional Chinese Medicine, based on which the types of syndromes consist of the syndrome of excessive dampness due to deficiency in spleen, syndrome of yin deficiency of liver and kidney, syndrome of the stagnation of Qi due to the depression of the liver and the syndrome of static blood blocking collaterals. Two doctors above the title of the associate professor in dermatology departments were invited to make a definite diagnosis of chloasma; and the practitioner of Chinese medicine was responsible for collecting the information on the four diagnostic methods of patients, filling in the questionnaire and analyzing the physical constitution and syndrome of patients according to the above criteria. The enumeration data adopts chi square test, Fisher exact probability test and Table R X C exact probability. Analyze the distribution regularities of the constitution types and syndromes and the dependency between the constitution types and syndromes.Results::â‘ The TCM constitutions of chloasma patients are: phlegm-dampness (29), Yang deficiency (14), damp heat (6), Yin deficiency (27), blood stasis (18), Qi depression (24) and special constitution (2). The distribution in different TCM constitutions: phlegm-dampness takes up the largest proportion, followed by Yin deficiency> Qi depression> blood stasis> Yang deficiency> damp heat> special constitution. The statistical results show that the differences of the ratio of different constitutions have a statistical significance (p=0.000<0.001).â‘¡The syndromes of chloasma patients are the syndrome of excessive dampness due to deficiency in spleen (49), syndrome of yin deficiency of liver and kidney (27) and syndrome of static blood blocking collaterals (18). The Distribution of syndromes:the syndrome of excessive dampness due to deficiency in spleen accounts for the largest proportion, followed by the syndrome of yin deficiency of liver and kidney> liver depression and Qi stagnation> syndrome of static blood blocking collaterals. The statistical results present that the differences of the ratio of different syndrome types are of a statistical significance (p=0.000<0.001).â‘¢Dependency between the constitution and syndrome:The phlegm-dampness shows obvious relativity to the syndrome of excessive dampness due to deficiency in spleen (p=0.000<0.001); the Yin deficiency constitution is highly related to the syndrome of yin deficiency of liver and kidney; the blood stasis is highly related to the syndrome of static blood blocking collaterals; and the Qi depression constitution shows evident dependency with the syndrome of liver depression and Qi stagnation (p=0.000<0.001).Conclusion:â‘ The phlegm-dampness, Yin deficiency and Qi depression types are found susceptible to chloasma. According to the idea of "preventive treatment of disease", it is suggested to guide the phlegm-dampness,Yin deficiency and Qi depression groups in health care and improvement of the constitution, in order to lower the incidence of chloasma.â‘¡The syndrome of excessive dampness due to deficiency in spleen is the major clinical syndrome type of chloasma patients. In the clinical treatment of chloasma, the chronic skin disease, it will improve the clinical efficacy by precisely recognizing symptoms and signs, handling the common syndrome type of excessive dampness due to deficiency in spleen based on the fundamental principle of invigorating the spleen to damp elimination.â‘¢The difference in the constitutions of the chloasma patients decides the difference in the clinical syndromes.In addition to the four diagnostic methods, the cure of chloasma should combine the assessment of the constitution and adopt different treatment methods for different constitutions, in order to improve the cure rate of the disease and improve the life quality of the patients.
