Objective:To investigate the characteristics of clinical and the influence on theprognosis of142cases of epithelial ovarian cancer.Methods:The clinical data of142cases of epithelial ovarian carcinoma patients whohad been treated in our hospital from January2005to December2009was retrospectively analyzed.We classified the all patients into four groups,≥70age,60~69age,50~59age, and<50age.To compare their clinical stage, pathological type, histological grade,postoperative residual tumor size of primary cytoreductive surgery,lymph node dissectin and transference and course of chemotherapy respectively,and to analyze its prognostic factors.All patients were followed-up by telephone and survival time was determinedfrom the date of surgery to date of the last follow-up or death.All data was statisticsllyanalyzed using SPSS13.0software.Results:1、The patients with age groups between50to59years are most commonly affected with32.4%response.The proportion in other3age groups are26.1%(60~69age),22%(<50age) and19.7%(≥70age).2、Clinical stage: In early stage(FIGO stage I),the proportion of patients with age less than50years was significantly higher than thepatients with age more than60years old(P<0.05),while the patients over60years oldwith advanced disease(FIGO stage III/IV) accounted for the more majority.There wassignificant statistically difference(P<0.05).3、Pathological type:Of all pathologicaltypes,the serous histotype was the most common type of ovarian carcinoma.The patientsover70years old with serous cancers were more than less than50years(P<0.05),while the patients less than50years old with endometrioid cancers accounted for themore majority(P<0.05).4、 Histological grade: In intermediate carcinoma, theproportion of patients with age less than50years was significantly higher than thepatients with age between60to69years old(P<0.05).In low-grade carcinoma, theproportion of patients≥50years old was significantly higher than that of<50years old,the difference has statistically significant(P<0.05).5、Postoperative residual tumorsize of primary cytoreductive surgery:In the removal of all visible tumor group, thepatients less than50years old accounted for the more majority compared with over60years old patients(P<0.05),while the patients over60years old with cytoreduction to>2cm residual disease had higher proportion(P<0.05).6、Lymph node dissectin andmetastasis:The patients over70years and between50to59years old accounted for themore majority compared with the patients less than50years old in no systematiclymphadenectomy(P<0.05).Of all the patients with presence of no node metastases,significant statistical difference between less than50age to over50years old wasnoted,and the patients less than50years old accounted for the more majority.7、Courseof chemotherapy:Less than50years old young women who completed chemotherapywere significantly more than over70age(P<0.05).70years old or more elderly womenhad higher proportion than less than60years old among patients who refused tochemotherapy.There was significant statistically difference.8、Logistic regression modelmultivariate analysis found that the prognosis of ovarian cancer had significantcorrelation with FIGO stage,postoperative residual tumor size, histological grade andpostoperative chemotherapy.Conclusion:1、The patients with age groups between50to59years are most commonlyaffected.2、The serous histotype was the most common type of ovarian carcinoma.Thepatients with later phase and poorly differentiated are inclined to the eldly.Among theoptimal cytoreductive rate,the systematic lymphadenectomy rate and the completionrate of chemotherapy,proportion of young women is better than the elderly.3、Theprognosis of ovarian cancer had significant correlation with FIGO stage,postoperativeresidual tumor size, histological grade and postoperative chemotherapy.
