Contrast Osteoporotic Thoracolumbar Vertebral Compression Fractures With Percutaneous Verte-broplasty Efficacy Of Conservative Treatment

Objective：To compare osteoporotic thoracolumbar vertebral compression fractures(OVCF) PVP surgical treatment with conservative treatment efficacy.Methods：From September1,2011to2013, January1, Author myself collectedOVCF patient data a total of72cases in second Department of Orthopedics of Linyi CityChinese Medicine Hospital bone, including the surgical group PVP surgical therapy of38cases, the surgical group of patients：The youngest was60years old, The oldest was91years old, Average is71years. Tell the condition and treatment programs for this group ofpatients,let them choose surgery by themselves, preoperative the MRI to determine thefresh vertebral and CT to determine the integrity of the risk of vertebral edge. To rule outother possible pathological causes of vertebral compression fractures.Conservative therapyconservative group Include a total of34cases, conservative group of patients：Age wasabout53-91years old, with an average of about70years old. All the patients correspondedthe inclusion criteria. All the patients chose conservative treatment after explained thedisease and the appropriate treatment options, also said to be able to actively cooperatewith functional exercises. All patients were followed up, and evaluated efficacy accordingto the JOA score and VAS, ODI score, kyphosis Cobb angle，vertebral height andvertebral height loss rate.Results：All patients were followed up. One day after PVP treatment JOA scoreincreased from12.84±2.64to22.74±2.26, the VAS score dropped to3.00±1.25from7.00±1.27, ODI score decreased from39.34±3.01to29.34±5.54, with more significant differences in the conservative group(P<0.05); one month,three months, JOA score andVAS scores of the two groups of patients, ODI score difference was not significant. Thepatients of the PVP group after treated one day, one week and one month,the pain reducedsignificantly,better than the conservative group. After three months,the JOA score, VASscores and ODI score of the two groups were no significant difference.In reducingkyphosis Cobb angle, to restore vertebral height and improve the rate of loss of vertebralheight, both group had no significant effect. Two cases of PVP group had occurred theleakage of bone cement, but there was no significant nerve compression, spinal cord injury,one case of peripheral venous leakage of bone cement did not cause serious pulmonaryembolism, neither do special processing. Two patients suffered bedsores and one casesuffered urinary tract infection of the conservative group. In the follow-up, there was noadjacent vertebral fractures in the two groups of patients.Conclusion：The Vertebroplasty can relieve pain quickly and effectively,stabilize thevertebral body, improve the quality of life, and the surgical trauma avoid the bedriddenserious complications, is one of the ideal way to treat OVCF. Conservative treatment canmake the same pain relief, lower costs, but the course of the disease is a long time., thepain is great, while there are many complications.
