Clinical Research Of Tonifying Qi And Yin, Activating Blood Circulation And Dispelling Turbidity Treating Diabetic Kidney Disease At Stage Of â…¢ To â…£(Qi And Yin Deficiency, Cloud-stasis Stagnation Syndrome

Objective:To explore the effects of the method of tonifying Qi and blood, activating Blood circulation and dispelling turbidity on the diabetic kidney disease at stage of III to IV (Qi and Yin deficiency, cloud-stasis stagnation syndrome) in the sides of TCM syndrome, renal funct-ion, glucose metabolism, lipid metabolism, blood rheology and inflammatory response and initially learn the mechanism of the method treating the diabetic kidney disease at stage of III to IV.Methods:To observe60diagnosed patients with diabetic kidney disease at stage of III to IV in the hospitalization and outpatient of Nephrology, Endocrinology Department in the affilia-ted hospital of Chengdu University of TCM, Who are differentiated by Chinese dialectical as Qi and Yin deficiency Cloud-stasis stagnation syndrome. According to randomized, control study, the60patients were divided into experimental and control groups,30patients in the test group of and the other30patients in the control group.The control group is given lifestyle intervention and conventionally western medicine, while the treatment of the test group is on the basis of the control group plus tonifying Qi and yin, activating Blood circulation and dispelling turbidity daily. The treatment course is2months. To observe changes of traditional Chinese medicine syndromes and symptom score, renal function, glucose metabolism, lipid metabolism, inflammatory mediators and blood flow and other indicators before and after treatment,lastly do statistical analysis.Results:the total effect scores:the test group total effective rate is79.3%, and the control group total effective rate is59.25%, so the total efficiency of the test group is better than the control group (P<0.01), with highly statistical significance.In terms of traditional Chinese medicine syndromes,two groups significantly reduce TCM syndrome (P<0.01), especially in the sides of dry mouth hi drink, fatigue, weak waist, edema, nocturia improvement of clinical symptoms the experimental group is better than the control group (P<0.05).In the control of blood pressure, the two groups can make the drop in the blood pressure (P <0.05), but there was no significant difference.(P>0.05). In the control of blood glucose, the two groups can be significantly reduced fasting blood glucose,2-hour postprandial blood glucose and glycosylated hemoglobin (P<0.01),but the test group lower fasting blood glucose than the control group (P<0.05), but in the control of2-hour postprandial blood glucose and glycosylated hemoglobin, there is no difference (P>0.05).In the control of proteinuria, the two groups may be reduced proteinuria (P<0.05), but the experimental group is better than the control group (P<0.01).In the improvement of kidney function, the two groups can be significantly reduced serum creatinine, blood urea nitrogen, to some extent, improve renal function (P<0.01), but the experimental group group is better than the control group (P<0.05).In the sides of Corrective lipids, the two groups can be significantly reduced TG, TC, LDL-C(P<0.01), increased HDL-C(P<0.05), but the experimental group reduced TC, TG signifi-cantly better than the control group (P<0.01)。 However there is no difference on the part of increased HDL-C,lowered LDL-C (P>0.05).In the improvement of blood rheology, both groups can improve significantly whole blood viscosity (high-cut, in the cut, low-cut5/s) and plasma viscosity (P<0.01), the experimental group is better than the control group (P<0.05).the experimental group can improve erythroc-yteaggregation index and hematocrit (P<0.05), but the control group can not(P>0.05).In the control of inflammation, the two groups could significantly reduce C-reactive protein control inflammatory response P (P<0.01), but the experimental group is better than the control group (P<0.01).Conclusion:Due to more common diabetic kidney disease at stage of III to IV in patients with Qi and Yin deficiency, cloud-stasis stagnation syndrome and my instructor’s many years of clinical experience, it establishes main treatment and prescription of tonifying Qi and yin, activating Blood circulation and dispelling turbidity. Here are some functions.①This method can improve the the diabetic kidney disease III to IV patients with tradition-al Chinese medicine clinical symptoms, especially in the dry mouth hi drink, fatigue, weak waist, edema, nocturia symptomatic improvement advantages.②This method can significantly lower in patients with diabetic kidney disease urinary albumin, serum creatinine and blood urea nitrogen and slows down the deterioration of renal function.③This method can reduce diabetic kidney disease in patients with glucose, blood pressure, especially for easier control of fasting blood glucose target level.And it can significantly reduce the role of the diastolic blood pressure.④This method can regulate lipid metabolism of patients with diabetic kidney disease, can reduce serum cholesterol, triglycerides, low-density lipoprotein,and mildly elevated high-den-sity lipoprotein.⑤This method can inhibit platelet aggregation, play a role in anti-thrombosis prevention and treatment of acute cardiovascular and cerebrovascular events, to some extent, the mort-ality rate and morbidity. can be reduced。⑥This method can reduce the the diabetic kidney disease inflammatory mediators and inhibit the inflammatory response, thus it can block renal fibrosis and protect residual renal function.
