Objective:1.To describe the current status of self-management of outpatients withhypertension;2.To evaluate the effects of a self–management intervention for outpatientswith hypertension;3.To explore the influential factors that affect the self-management.Methods: A randomly controlled trial was taken, to recruit a total of282patients withhypertension in outpatient department from two grade three general hospitals in XinjiangUrumqi. Patients with hypertension were randomly divided as intervention group andcontrol group, there were141patients in each group. The patients in the interventiongroup received a self-management intervention, the patients in the control group receivedthe general health education. Ability of self-management, illness-cognitive, self-efficiency,blood pressure, body mass index, waist circumference between two groups were comparedafter6-months intervention. Meanwile, to analysis the influential factors that affect theself-management after the intervention. Results:1. Baseline data: The level of theself-management of outpatients with hypertension were at moderate level.2. Result of twodifferent intervention:(1) The comparison of data before and after6-months intervention:1) In intervention group there were increase of the level of the diet, adherence tomedication, daily activities, overall level of self-management, illness-cognitive, self-efficiency and there were decrease of the level of the blood pressure, body mass index,waist circumference (P<0.01).2) In the control group there were increase of the level ofthe adherence to medication, illness-cognitive and there were decrease of the level ofsystolic blood pressure, body mass index (P<0.05).(2) The comparison of difference ofbetween two groups before and after6-months intervention: there were increase of level ofthe diet, adherence to medication, daily activities, overall level of self-management, illness-cognitive, self-efficiency and there were decrease of the level of the blood pressure, waistcircumference in the intervention group were better than the control group (P<0.01), but social psychology, body mass index without improvement in the intervention group werethan the control group (P>0.05).3. Analysis result of influential factors of self-management of outpatients with hypertension:(1) Single factor analysis shows gender, race, age,education level, duration of disease, complication, self-efficiency, illness-cognitive aremajor influential facors self-manage ment for outpatients with hypertension.(2) Thevarious dimensions of the factors analysis shows illness-cognitive, race, self-efficiency,age are major influential facors self-manage ment for outpatients with hypertension.Conclusions: The level of self-management of outpatients with hypertension were in themiddle level. Self-management intervention can effectively improve unhealthy lifestyleand promote the level of self-management, and decrease the level of blood pressure foroutpatients with hypertension. Because of the model with simple operation and low cost, itis worth to apply in the health management of hypertention.
