Objective:To focus on strengthening the inputs and capacity of public health, primary health and basic medical care, and by developing targeted interventions, the goal of equalization of basic public health services can be achieved. And finally the folks get sick later, less, lighter and even none. This paper focuses on the improvement and promotion of the equalization of basic public health services among urban and rural residents to analyze the status of implementation of the equalization of basic public health services recently in Tianjin, and to find out the major problems in the process of equalization. In the end, the paper puts forward operational and propagable policy recommendations which conforms to Tianjin’s social economic development level.Methods:Gini coefficient and Lorenz curve are adopted to analyze the equity of basic public health services of urban and rural residents. By expert consultation, it establishes the evaluation index system of basic public health service equalization in line with the actual situation in Tianjin. It also assesses comprehensive evaluation of regional and Lasted analysis progress of basic public health services in Tianjin, with principal component analysis.Results:Basic public health services in Tianjin has achieved significant improvements in many aspects such as the overall policy making, funding, capacity, supply and so on. However, the development of basic public health services between urban and rural areas is still imbalance. Firstly, despite of the raise of basic public health services funding, the proportion remains in a low level and there’s regional differences of budget allocation so that the counterpart funding can not be achieved timely and fully. Secondly, staff in basic health institutions are inadequate and weak of service. Regional distribution of personnel is imbalance. Thirdly, the quality of some basic public health service items need improving, and the gap of the amount of items between urban and rural areas is significant. Fourthly, the incidence rate of non-communicable diseases tend to rising, and the outstanding urban-rural differences of coronary heart disease morbidities suggests the enhancement of related items in rural areas. Finally, the degree of equalization of basic public health services in Tianjin shows a positive trend year by year as a whole, while some of the projects still need to be further improved.Conclusion:Basic public health services are important parts of health work recent years in Tianjin, which have been on the top nationally. The study recommends the government to strengthen the financial support on public health and basic public health services, and to improve the financing payment structure. To improve basic health services in urban and rural areas with the help of community health service system, consisted of community and township health center as the main body, and combining professional public health agencies. According to the requirements to achieve the equalization of basic public health services of new medical reform, Tianjin will create a new situation of basic public health services.
