Objective: To explore the methods of microsurgery on glioma.Methods:Retrospectively reviewed the data of44patients with glioma excisedunder microscopic in our department the past4years, summarized the surgical methods,explored the surgical skills, explored the advantage and clinical effect of microsurgicaltechnique.Results: All the44patients underwent microsurgical therapy.34patients’ tumorswere completely excised, accounted for71.4%;10patients’ tumors were gross-partiallyresected, accounted for28.6%. After the microsurgical therapy,29patients recoveredwell;15patients experienced improvement;5patients appeared short disturbance ofconsciousness;5patients appeared contralateral temporarily muscle strength declinedand returned to normal in about7days after giving symptomatic treatments withdehydration, hormone and neurotrophy. No surgical death, intracranial infeion andleakage of cerebrospinal fluid or subcutaneous hydrops occurred.10patients withhemiparalysis before the operation improved their sensory function and motor function.44patients were followed from5months to24months postoperatively.13patientsrelapsed, relapse rate accounted for29.5%;9cases died,21patients resumed a work,8patients could live independently.10relapsed patients received reoperation,3relapsedpatients refused reoperation.10patients with hemiparalysis after the operation werefollowed half one year,4patients recovered,6patients still had hemiparalysis butimproved.Conclusion: In the wake of the establishment and development of themicroneurosurgery and microsurgical technique gradually maturing, neurosurgerytreatment level obviously improved, microsurgery on lots of the brain tumors madegood clinical effect. Through microsurgery, the glioma could be completely excised,operative bleeding reduced, important vascular could be keep, neural function could be protected furthest, the normal brain tissue damage reduced; after the microsurgery,symptoms were significantly improved, patients recovered fastly. Therefore,microsurgery could obviously improve the therapeutic effect, prolong the life time ofpatients, improve the life quality of patients, decrease recurrence rate and mortality. Themicroneurosurgery has been the first choice of surgical operations for glioma, worthy ofclinic application.
