The Analysis Of Cryptogenic Organizing Pneumonia And Nonspecific Interstitial Pneumonia

Objective: To analyse cryptogenic organizing pneumonia and nonspecificinterstitial pneumonia clinical characteristics, auxiliary examination and pathology,exploring the cryptogenic organizing pneumonia and nonspecific interstitial pneumoniaclinical identification methods.Methods: To choose The First Affiliated Hospital of DaLian medical Universityfrom January2009to December2011through pathology diagnosed4cases ofcryptogenic organizing pneumonia,2cases of nonspecific interstitial pneumonia as theresearch object, and record the patient’s clinical data, lung CT and pathological material.Results:1case contact with dust history,1case contact welding history,1casecontact with asbestos history,1case have penicillin and cephalosporin drug allergyhistory, the rest not have all kinds of industrial dust and plant dust exposure, no drugallergy history, and smoking is not very close relationship.6cases in hospitalizedpatients with age range of51-73years old, an average of60.17±8.52,5cases of female,male1cases, female ratio is higher,the most common symptom like cough、 sputum6cases, shortness of breath5cases, at beginning of the disease2patients have fever,2patients have clubbing,4cases have Velcro rale, more in NSIP；1case has lungsmoist rale；1case lungs auscultation have not positive symptoms；4cases blood gasanalysis suggests mild hypoxia;5cases have restrictive ventilation dysfunction anddispersion function is reduced in which1cases do not lung function testing；Lung CTprompt：NSIP lung CT mainly for random distribution, show ground glass change to thesubpleural distribution is given priority to, some of which are grid change，Only onecase a honeycomb-like change；COP main show more diversified CT, for patchy,wander the patchy shadow, small flake shadow, and along the bronchi, the distributionof the vascular bundle primarily, may be associated with ground glass change. One case a honeycomb-like change. Pathology COP group microscopically machine ofOrganizing pneumonia, NSIP group microscopically lung tissue mucosal inflammationchange, lung interstitial fibre hyperplasia.Conclusion:1.COP and NSIP clinical symptoms, signs, laboratory examination,pulmonary function have different degree of similarity.2. Imaging findings anddifferential diagnosis significance, but cannot be used as the basis, must rely onpathology.3.Glucocorticoid therapy more effective, to individual dose.
