Depression and anxiety are closely related to hypertension. On one hand, depression and anxiety influence the occurrence, development, treatment and prognosis of hypertension throughout. Depression and anxiety are independent risk factors for hypertension. Depression or anxiety affects Circadian rhythm of blood pressure and left ventricular hypertrophy, and causes change of some biochemical indicators, and so on, leading the aggravation of hypertension. Combined with depression or anxiety will reduce compliance and effect of antihypertensive treatment. Depression and anxiety can both increase the risk of cardiac death, and influence the prognosis of patients with hypertension. On the other hand, hypertension can also cause depression and anxiety, there is a high incidence of depression or anxiety in patients with high blood pressure.some antihypertensive drugs such as reserpine, can lead to depression. To high blood pressure patients merged with depression or anxiety, both hypertension treatment and treatment of depression and anxiety should be used. A large number of clinical studies have shown that antihypertensive therapy combined with anti-depressant or anti-anxiety treatment of drug and psychological treatment, has the best antihypertensive effect, and can effectively improve the patient’s body and mental symptoms, and improve patient’s quality of life and prognosis. Traditional Chinese medicine, for high blood pressure, depression, anxiety, has a unique understanding, also have their own characteristics and advantages in treatment. Compared with the pure western medicine or Chinese medicine treatment, Combination of traditional Chinese and western medicine can increase the efficacy and reduce side reaction through the complementary advantages, and is the best choice of treatment for high blood pressure, depression and anxiety. Review part summarized the relationship between depression, anxiety and high blood pressure, the features and the treatment of high blood pressure patients with depression or anxiety, and research progress on the etiology and pathogenesis, syndrome differentiation, treatment, combined Chinese and western medicine for hypertension, depression and anxiety in traditional Chinese medicine.Second, clinical investigationPurpose: through the clinical investigation cases, analyze the onset of depression, anxiety in patients with high blood pressure and relevance to the TCM syndrome type.Methods:in the form of a questionnaire to collect rating data of51cases of patients with high blood pressure,such as the general conditions, social factors, HBP associated condition,Chinese medicine dialectical and emotional situation, using the Excel software and SPSS17.0statistical software for statistical analysis.Results:①incidence of depression in patients with high blood pressure was significantly higher than control group; Incidence of anxiety in patients with hypertension is significantly higher than control group;②The SAS score of women, over60years old, Irritability and people with low educational level is higher than men, under60years old, mild,and people with high education level. The SDS score of women, over60years old people is higher than men, under60years old.③The age, low educational level are risk factors for hypertensive patients with anxiety. Female is a risk, factor for hypertensive patients with depression.④Among the TCM syndrome types of hypertension patients combined with anxiety, Yin and Yang deficiency syndrome take the biggest place;⑤Among the TCM syndrome types of hypertension patients suffered with depression, Liver fire excessive syndrome and Blood stasis syndrome are very common.⑥Hypertension patients of both Yin and Yang deficiency syndrome have the highest incidence of anxiety. Hypertension patients of blood stasis syndrome have the highest incidence of depression.Conclusion:①The incidences of anxiety and depression of hypertensive patients are higher than that of the control group.②over60years old and low educational level are risk factors for hypertensive patients with anxiety. Female is a risk factor for hypertensive patients with depression.③Hyper tens ion patients of both Yin and Yang deficiency syndrome have the highest incidence of anxiety. Hypertension patients of blood stasis syndrome have the highest incidence of depression. |
