Objective:①To explore Chinese Medicine syndrome trait of Chronic Cerebral Circulation Insufficiency;②By observing the change scores of the Chinese and Western Medicine scales To objectively assess the treatment effect of Reinforcing Kidney and replenishing essence method for CCCI;③To investigate Brain CT perfusion for assessing the Brain blood perfusion of Patients with CCCI,and the change of the brain blood perfusion before and after Chinese Medicine treatment.Methods:Thirty patients with CCCI underwent Brain CT or MRI,TCD, Carotid artery ultrasonography. All patients take self-created Chinese Medicine prescription "Reinforcing Kidney and replenishing essence granules" every morning and night for3months(Ingredient:wolfberry fruit,Tuber Fleeceflower Root,Tall Gastrodia Tuber,Prepared rhizome of Adhesive Rehmannia, Desertliving cistanche, Cornus officinalis, Motherwort fruit),and both at first day and after3months take Chinese Medicine Symptom Score Scale,Hamilton Depression Scale(HAMD), Hamilton Anxiety Scale (HAMA), Minimum Mental State Examination (MMSE),Montreal Cognitive Assessment (MoCA), Pittsburgh Sleep Quality Index(PSQI). At first day all patients do TCD, Carotid artery ultrasonography, Brain CT or MRI,Routine blood test,Urine routine test, Stool routine test,Biochemical tests. Ten patients do Brain CT perfusion to assess the brain blood perfusion at the first day, and after3months two of them also do CTP to observe the change of the brain blood perfusion after Chinese Medicine treatment.Using SPSS17.0statistics software to process the data.Results:①Thirty patients with CCCI all appeared Marrow deficiency symptom(100%);83%of them appeared Qi deficiency symptom;70%appeared Yang deficiency symptom;63%appeared Phlegm symptom;60%appeared Blood deficiency symptom.Heat symptom,wind symptom, blood stasis symptom and Yin deficiency symptom were not obvious.②The result of clinical treatment effect of Thirty patients with CCCI:Significant efficiency rate is34%; effective rate is56%; invalid rate is10%; total effective rate is90%; The result of symptom treatment effect:Significant efficiency rate is3%; effective rate is80%; invalid rate is17%; total effective rate is83%.③By the paired sample T test to analysis the scroes of Chinese Medicine Symptom Score Scale, Hamilton Depression Scale(HAMD), Hamilton Anxiety Scale (HAMA),Minimum Mental State Examination (MMSE),Montreal Cognitive Assessment (MoCA);Pittsburgh Sleep Quality Index(PSQI) before and after the Chinese Medicine treatment,P<0.05,there were statistical significance.④The CTP result of ten patients with CCCI at first day is:Compared with the healthy side CBF,the sick side CBF failed,by the paired sample T test to statistics the data,P<0.05, there were statistical significance; Compared with the healthy side CBV,the sick side CBV failed, by the paired sample T test to statistics the data,P>0.05, there were not statistical significance. Compared with the healthy side MTT,the Sick side MTT prolonged,by the paired sample T test to statistics the data,P>0.05,there were not statistical significance.⑤Two patients with moderate or severe stenosis of right middle cerebral artery. compared with the CTP result on the first day,the CTP result on3months laterthe ratio of rCBV,rCBF and rMTT failed Compared to the first day;All CT perfusion parameter Images obviously displayed that perfusion was improved.Conclusion:The Clinical symptoms of Chronic Cerebral Circulation Insufficiency is Consistent with Marrow deficiency symptom of Chinese Medicine.The symptom is intermingled deficiency and excess,the deficiency mainly shows Qi deficiency,Yang deficiency, Blood deficiency,excess mainly shows Phlegm.The treatment effect of Reinforcing Kidney and replenishing essence method for CCCI shows evidently,which can improve the symptoms of CCCI,also the accompanied symptom depression, anxiety,cognitive function decline,insomnia.CTP can accurately assess the brain blood perfusion of patients with CCCI.in addition,CTP can also be used for assessing the change of brain blood perfusion before and after drug treatment. |
