Based On The Modern Literature Of Cirrhosis Of The Liver Syndrome, Syndrome Elements Distribution Characteristics And Often Witness To Explore Corresponding Relationship With Symptoms

Liver cirrhosis is a common chronic, progressive or diffuse disease due to different etilogical factors, it is diffuse hyperplasia of live fibrous tissue based on wide degeneration and necrosis of hepatocytes and the formation of regenerated nodule and pseudolobule, which cause the damage of normal liver lobule structure and blood vessel, thus liver cirrhosis is a high incidence, more complications and high death rate disease with long course, progressive sicker and difficult to cure. The Traditional Chinese Medicine (TCM) has an advantage in terms of alleviating the symptoms, reducing complications and death rate. The current research on liver cirrhosis focuses on the the summary of clinical experiences and curative effect, there is a lack of syndrome standardization, which restricts the development of the clinic and research work of cirrhosis. Objective:This study is to summarize the distribution characteristics of the common syndromes, symptoms and syndrome factors by retrospectively studying of the latest20years literature, so as to provide the evidences for building and completing the syndrome differentiation standard.Methods:1. Collect the literature about liver cirrhosis of the latest20years by electronic search and define the proper ones.2. Make a standard for the names of syndromes and symptoms.3. Built the data base of syndrome and symptoms of liver cirrhosis by Epidata3.0.4. Split the standard syndrome names into disease-location syndrome factors and disease-nature syndrome factors according the definition and the classification principle of syndrome factors.5. Statistic analysis of the frequency of syndrome, syndrome factors and symptoms by SPSS17.0.Results:We got814literature and214were qualified for my research. The statistic results are as follows:1. The common syndrome of liver cirrhosis:(1)In90syndrome types, the top10of frequency are yin deficiency of liver and kidney syndrome, yang deficiency syndromeof both spleen and kidney, dampness-heat syndrome, blood stasis syndrome, dampness-retention due to spleen deficiency syndrome, spleen-deficiency-induced dampness retardation syndrome, stagnation of qi and blood stasis, stagnation of liver qi, stagnation of fluid-dampness syndrome, retention of dampness due to qi stagnation syndrome.(2)The liver cirrhosis can be divided into compensatory stage and decompensatory stage. The syndrome distribution is as follows:①There are36types of syndrome in compensatory stage, the frequency of syndrome are below10%.②There are9types of syndrome in decompensatory stage, the syndrome frequency over10%are yang deficiency of spleen and kidney syndrome, yin deficiency of liver and kidney syndrome, stagnant and jamming damp-heat syndrome.2. The distribution of common syndrome factorsThere are28syndrome factors in liver cirrhosis, in which10are disease-location syndrome factors and18disease-nature factors. The frequency over10%of disease-location syndrome factors are3:spleen, liver and kidney; in disease-nature factors, the frequency over10%are6: blood stasis, qi stagnation, yin deficiency, qi deficiency, dampness-heat and yang deficiency. There are20types of syndrome factors in compensatory stage, in which6types are disease-location syndrome factors and14disease-nature factors. The frequency over10%of disease-location syndrome factors are3:spleen, liver and kidney; in disease-nature factors, the frequency over10%are5:qi stagnation, qi deficiency, yin deficiency, blood stasis, yang deficiency.There are25types of syndrome factors in decompensatory stage, in which9types are disease-location syndrome factors and16disease-nature factors. The frequency over10%of disease-location syndrome factors are3:spleen, liver and kidney; in disease-nature factors, the frequency over10%are5:blood stasis, qi stagnation, yin deficiency, yang deficiency and dampness-heat.3. The distribution of common symptoms(1) The overall distribution of symptoms The number of liver cirrhosis symptoms are253, include the general symptoms, the tongue manifestation and pulse manifestations, et, al. There are206types of general symptoms,32tongue manifestations and15pulse. The frequency of symptoms over3%are:①The4general symptoms:abdominal distension, abdominal swelling, less urination and loose stool.②12tongue manifestations:red tongue, light-white tongue, purple tongue, dark tongue, deep red tongue, red tongue without fluid, ecchymoses tongue, plump tongue, greasy fur, white fur, yellow fur, thin fur;③8pulse manifestations:stringy puls, thready pulse, rapid pulse, deep pulse, hesitant pulse, slippery pulse, weak pulse, moderate pulse. (2) The symptom distribution in two stages:the top10frequency of general symptoms and top3frequency of tongue and pulse manifestations are defined as common symptoms. The distribution are as follows:①The compensatory stage:loose stool, spiritlessness, weakness, abdominal distention, no appetite, oppression in chest, spider angioma, flatulence threat, dry mouth, bitter taste; white fur, red tongue, greasy fur; stringy pulse, thready pulse and rapid pulse.②The decompensatory stage:less urination, abdominal distension, abdominal swelling, loose stool, edema of lower extremity, blue veins appear in the abdomine, fatigue, dry mouth, weak strength; white fur, red tongue, greasy fur; stringy pulse, thread pulse and rapid pulse.4. The correspondence relationship between syndrome and symptom(1)Common syndrome symptoms/signs of frequency analysis Take the frequency of the largest10kinds of syndromes of cirrhosis of the common card waiting in the various syndromes take the frequency of the top10symptoms for the common symptoms of cirrhosis common syndrome corresponding symptoms.The frequency distribution see the article.(2)Common syndrome-related symptoms/signs of logistic regression analysisBy binary logistic regression analysis, try to find out the check against climate contribution symptoms (specifically, see article)Conclusions:1. The common syndrome types in liver cirrhosis are the syndrome of yin deficiency of liver and kidney, the syndrome of yang deficiency of spleen and kidney, the syndrome of stagnant and jamming damp-heat, the blood stasis syndrome, the syndrome of spleen deficiency due to liver qi staganation, the dampness-retention due to spleen deficiency syndrome, stagnation of qi and blood stasis syndrome, stagnation of liver qi, stagnation of fluid-dampness syndrome and retention of dampness due to qi stagnation syndrome.2. The common disease-location syndrome factor of liver cirrhosis are spleen, liver and kidney; the common disease-nature syndrome factor of liver cirrhosis are blood stasis, qi stagnation, yin deficiency, qi deficiency, dampness-heat and yang deficiency.3. The terminology use of cirrhosis syndrome and symptom is widely existed.4. In the study of modern literature, the reporters don’t often use the diagnostic standard.5. The syndrome of cirrhosis is complicated, but it can be simple when using the syndrome factors, which can also provide evidence for the building and completiong the syndrome differentiation standardization of cirrhosis. 6. Frequency analysis and Logistic regression analysis results in line with cirrhosis common syndrome syndrome characteristics.
