BackgroundThe township hospitals are the main health resource in rural area. They are the great node of preventing and controlling infectious disease in rural area, and also the first defense line for rural residents to prevent the infectious disease. Jinan has65township hospitals in2010. These hospitals reported803valid infectious disease cards, the promptness of reporting is99.75%. But there are also many problems in infectious disease reporting in township hospitals, such as weak management, bad training and low quality in infectious disease reporting.ObjectiveTo know the status of infectious disease reporting of township hospitals in Jinan, and to further assess the reporting quality of infectious disease. At last, to make specific recommendations for the problems.Materials and methodsThe study object is township hospitals in Jinan rural area. The township hospitals’ staff, management and hardware for infectious disease reporting are included in the study. We applied census method to investigate these hospitals. The database was setup by Excel2003software, and analyzed by SPSS for Windows13.0in unconditional logistic regression.Results1.65township hospitals have101staff who engaged in infectious disease report.86.14percent of them have a college degree and below. The proportion of public health prefesstionals is very low, only32.67%.70.30percent of the staff have junior titles.56.92percent of the township hospitals have no public health professtional.29.23percent of them have no staff who have college degree and above.67.69percent of them have no intermediate title and above who engaged in infectious disease reporting.2.84.62percent of the township hospitals have broadband network for infectious disease reporting.98.46percent of township hospitals’computer operating system is Windows XP. And93.85percent of township hospitals’computer memory is512M and above.3.41.54percent of the township hospitals gave a wrong answer of "notifiable infectious disease reporting time limit".90.77percent of them gave a wrong answer of "infectious diarrhea diagnostic criteria of clinically diagnosed cases" and "acute bacillary dysentery suspected cases of diagnostic criteria".66.15precent of them correctly answered four questions.4. From2009to2011, the pass rate and timely rate of infectious disease report cards of the township hospitals in Jinan is relatively high, more than99percent. The65township hospitals have780reporting months every year, the number of zero reporting month is543in2009,528in2010,404in2011, the proportion of the whole year is69.62%,67.69%and51.79%, respectively.5. There are four factors which have significant differences among the counties which have different economic development level. The four factors are whether one person is responsible for the management of infectious disease reporting, the answer of infectious disease diagnostic and report knowledge, zero reporting months, and the pass rate and timely rate of infectious disease reported cards.6. There are three factors which are mainly affect infectious disease reporting quality of the township hospitals in Jinan rural area. The three factors are the acedemic degree of infectious disease reporting staff, the number of diagnosised infectious diseases, and the medical staff’s knowledge of infectious disease diagnostic and reporting.Conclusion and suggestion1. The reporting quality of infectious disease of the township hospitals in Jinan rural area is low. The staff structure of infectious disease reporting is not reasonable. The medical staffs knowledge and abilities of infectious disease diagnositc and reporting is lacking. And the hardware facilities can meet the need of infectious disease reporting.2. There are differences in the reporting quality of infectious disease among the township hospitals which belong to different economic development level.3. The factors which affect the reporting quality of infectious disease in Jinan rural area are the acedemic degree of infectious disease reporting staff, the number of diagnosised infectious diseases, and the medical staffs knowledge of infectious disease diagnostic and reporting.The result suggests the government to increase the investment for the infectious disease reporting to support personnel and equipment costs. Suggests the health bureaus and the township hospitals to improve the staff structure of infectious disease reporting and the personnel quality. Suggests the twonship hospitals to enhance the effectiveness of training of infectious disease reporting, to improve the medical staffs diagnostic capablities of infectious disease, to effective implement the management measures of infectious disease reporting. Play the role of rural doctors in infectious disease reporting to reduce the infectious disease underreporting, to low the rate of monthly zero reporting. Suggests the township hospitals to improve the work efficiency by the means of information. By applying the measures above, to improve the reporting quality of infectious disease of the twonship hospitals in Jinan rural area.
