BackgroundAcrylamide (acrylamide, AM, CH2=CHCONH2), is a toxic, odorless white crystalline substance. Acrylamide is mainly used for the production of polyacrylamide. The latter is stable and is non-toxic. Polyacrylamide are widely used in petroleum and mining, tunnel construction, paper, sewage treatment, the production of paint, metal coatings and adhesives. China is the world’s largest producer of polyacrylamide. Polyacrylamide production develops with more than10%of the annual growth rate. The enterprises of production of acrylamide monomer are becoming more and more, but levels of prevention and control work is still very weak and poisoning accident are happening ceaselessly. The incidence of acrylamide poisoning is increasing. Acrylamide poisoning becomes occupational disease prevention focus. Acrylamide poisoning on central and peripheral nerves are damaged, there is no specific antidote. Most hospitals adopt traditional treatment regimens, they mainly use vitamin B class, energy mixture drugs. Treatment result is unsatisfactory, and the patients long recovery time. A minority of patients left limb numbness and weakness, gait and other symptoms. Clinical studies on acrylamide poisoning treatment conducted by domestic and foreign medical experts are reported no more. The acrylamide poising treatments need further exploration and research. ObjectiveTo analyze the clinical data and treatment options of60patients with acrylamide poisoning and observe patients with acrylamide poisoning during the treatment of the condition change, investigate the effect of acrylamide poisoning more effective treatment regimens, improve the treatment effect.MethodsClinical data of72cases from Jun1992to Jun2012, should be recorded, including name, age, gender, marriage, work, address, telephone, admission time, poisoning time and dosage, clinical manifestation, examinations, etc. The examinations include electromyography determination, blood, urine, stool routine, liver function, kidney function, chest X-ray film, head HRCT. We adopted comprehensive treatment scheme. The time points of observation are as follows:①mild poisoning:the day immediately after admission, the first month, second months, the third month after the end of poisoning or13weeks after discharge;②mnoderate poisoning:the day immediately after admission, the first month, second months, the third month,the fourth month after the end of poisoning or13weeks after discharge;③severe poisoning patients: the day immediately, fifth month, sixth months after admission, the first month, second months, the third month,the fourth month,the fifth month, the sixth months after the end of poisoning or13weeks after discharge.ResultsAmong the72cases, there were62male patients and10female patients. They were aged from17to57, whose average age was32.5. Contact length of service is20days to6.7years. Poison is mainly through the skin, mucosa absorption, a small amount of absorbed via the respiratory. Mild poisoning patients were given2-3months of treatment, moderate poisoning patients were given3-4months of treatment, patients with severe poisoning were given4-6months of treatment, symptoms, signs were disappeared, achieve clinical cure. We adopted comprehensive treatment scheme, and the patients with symptoms, signs and fast recovery. The treatment time is shortened for1-2months.ConclusionThis study analyzed clinical characteristics of72patients with acrylamide poisoning. They were treated with an appropriate and systematic therapeutics, by setting Perfect time points of observation and observing the results of examination. For comprehensive treatment scheme, we emphasize the following points:①choose the rat nerve growth factor, nerve nutrition gangliosides as first-line drugs;②give a small dose of glucocorticoid therapy early;③use tetramethylpyrazine hydrochloride of traditional Chinese herbal medicine;④use energy mixture;⑤reasonable hyperbaric oxygen therapy;⑥Physiotherapy, acupuncture, rehabilitation training;⑦According to EMG, We adjusted treatment. With guidance from this study, an evidence-based treatment may benefit patients with acrylamide poisoning.
