Objective To Compare the clinical effect of Metallic stent and LaparoscopicCholedochoduodenostomy for palliation of malignant obstructive jaundice.Methods During the period from June2009to December2013,32patientswith malignant obstructive jaundice underwent laparoscopicCholedochoduodenostomy and28patients underwent Endoscopic Metal BiliaryEndoprothesis were included,The parameters analyzed were the total bilirubin,operative time,duration of postoperative hospital stay, postoperative fastingtime,cost performance, prevalence of early and late complications, and prognosis.Results The rates of procedural and therapeutic success were excellent withboth palliative treatments.There was no a statistically significant difference in thetotal bilirubin and cost performance.With laparoscopic bypass, there was alower prevalence of late complications.With metallic stents, there was shorteroperative time,duration of postoperative hospital stay, postoperative fasting time,a lower prevalence of late complications,but a higher prevalence of latecomplications.Conclusion Laparoscopic choledochoduodenstomy is an effective method torelieve biliary obstructionis with low late morbidity and a low risk of recurrenceof jaundice.With a significant shorter operative time,duration of postoperativehospital stay,and postoperative fasting time,EMBE can rival the laparoscopiccholedochoduodenstomy. |
