Research On Care And Needs Assessment Instuments For Patients With Traumatic Brain Injury

Objective1．Introduce traumatic brain injury patients’care and needs scale ofEnglish version from Australia,translate and revise its entries, form a CANSscale of Chinese version and local trait.2．Design The Chinese CANS scale training course.3．Test its reliability and validity preliminarily,in order to help medicalworkers working in the community or clinical rehabilitation centers evaluateTBI patients’ care type and needs level conveniently andeffectively.Formulate individualized rehabilitation nursing measures for TBIpatients in the recovery period and give their main care givers guidance,beconvenient for Rehabilitation therapists and nurses to make classification management.So that the training course of the Chinese version CANS scalebeing made into VCD in rehabilitation center and provides the theory basisfor promotion in future.Method1．Adopt introduce program on international general scale, that is the"translation-rev-erse translation-cultural harmony" method, introduce TBIpatients care and needs assessm-ent tools to our country and revise itsentries by consulting experts and making preliminary test. Form a CANSscale of Chinese and local trait.2．By means of documentation to design training course content onCANS scale of Chinese version and teaching methods and by case methodand situational approach in teaching, analysing, discussing and swapping etc,training raters(rehabilitation nurse or rehabilitation therapist) who use theCANS scale of Chinese version. Make sure raters correctly understand themeaning of CANS scale of Chinese version and its operating methods.3．Adopt the convenience sampling method, extracte90light, and severeTBI patients who are in the recovery period as the research object, from a community health service center, two level-3hospital rehabilitationdepartment and a home private rehabilitation hospital in Tai’an City. Firstly,training raters(rehabilitation nurse or rehabilitation therapist) who use theCANS scale of Chinese version, then contrapose research objects,assessingthe level of care and support needs by Chinese CANS scale as research tools.Also note the research object’s gender, age, occupation, education, injuryreason, wounded date, losing memory time after injury and other basicmaterials. Data are recorded into SPSS17.0statistical software for furtherstatistical analysis, so as to test the reliability and validity of research toolspreliminarily.Results1．Introduce and revise care and support needs assessment tool suitablefor our country’s TBI patients, this tool includes24evaluation items and eightgrade levels.2．Through training the operation of raters who use Chinese version ofCANS scale, they can grasp and evaluate TBI patients’ care and supportdemand level accurately and objectively. The research tool is feasible. 3．Test the Chinese version CANS scale’s reliability and validity(1) The reliability and test-retest reliability of Chinese version CANSscale’s raters(between rehabilitation therapists and rehabilitation nurses, theresearcher): correlation coefficient of each item of Chinese version CANSis0.813~0.993（P<0.001）, The Cronbach’s α coefficient of total score is0.934（P<0.001）, retest correlation coeffi-cient is0.818（P<0.001）, retestcorrelation coefficient of each item is0.723~0.862. Before and after the testdata of heavy match is t-test, the difference was not statistically significant (t=0.347, P>0.05), the scale of the total score Cronbach ’s α coefficient is0.830(P<0.05).Chinese version CANS scale’s raters’(researcher, rehabilitationtherapists and rehabilitation nurses, the object of study, the research object’smain caregivers) agent reliability:The researcher and medical workers, theresearch object’s caregivers, raters’ correlation coefficient is0.931,0.721and0.521separately. Medical personnels and the researcher,caregivers, the objectof study’s correlation in the group is low:0.518,0.506,0.721. The researchobject’s caregivers and research object’s correlation in the group is0.851. （2）Chinese version CANS scale’s validity: Experts on the contentvalidity evaluation’s content validity index S-CVI/Ave is0.90. The entries ofChinese version CANS scale after turn model, the every factor loading factorshaft affinity-disaffinity relationship is clear. Chinese version CANS scaleand FIM scale evaluation level:rs=0.867(P <0.001).ConclusionThe TBI patients care and support needs assessment tool researched bythis task has simple language and short test time, the clinical raters and TBIpatients accept it easily. After the preliminary reliability and validity test, theChinese version CANS scale is proved to be scientific,reasonable and stable.
