| Objective 1.The purpose of this study is to translate the Pressure Ulcer Risk Primary or Secondary Evaluation Tool(PURPOSE-T)and establish a Chinese version of the pressure injury risk assessment tool,assess its reliability and validity in Chinese hospitalized patients.2.To explore the use of the Chinese version of the PURPOSE-T by clinical nurses,in order to provide scientific and practical guidance for the clinical application of the Chinese version of the PURPOSE-T.Method 1.We used the PURPOSE-T translation guide provided by the original author to localize the English version of the stress injury risk assessment tool.From November 2017 to March 2018,230 inpatients from Tianjin Third Central Hospital were selected to test the reliability and validity of the Chinese version of PURPOSE-T.Content validity and convergent validity were used to evaluate the validity of the scale.Inter-observer reliability and test-retest reliability were used to test the reliability.2.Using the descriptive phenomenological method in qualitative research and the purpose sampling method,the clinical nurses who participated in the first part of the study were selected as the research objects.The interviews were entered into the qualitative analysis software Nvivo 11.0.The interviews were collated and analyzed according to Giorgi data analysis principles and steps.Finally,a descriptive explanation of the experience of clinical nurses in applying the Chinese version of stress injury risk assessment tool was formed.Results 1.(1)The Chinese version of PURPOSE-T is formed after the Chinese cultural adjustment of the English version of PURPOSE-T.The tool is divided into three parts,including 25 items,involving 12 risk factors of pressure injury.The number of items is consistent with the source scale.(2)Validity of the scale: The results show that the content validity index(S-CVI)of the scale is 0.98,and the content validity index (I-CVI)of each item is 0.83-1.00.The results of convergent validity showed that the phi correlation coefficients of Chinese version PURPOSE-T with Braden scale and Waterlow scale were 0.781 and 0.777(P<0.001).The correlation coefficients of movement status,perception and response,nutrition,autonomous movement analysis,moist items and Braden scale items ranged from 0.605 to 0.877(P < 0.001).The correlation coefficients of nutrition,BMI,skin status items and Waterlow scale items ranged from 0.599 to 0.887(P < 0.001).(3)Reliability of the scale: The results of inter-observer reliability showed that the consistency kappa coefficient and weighted kappa coefficient were 0.798 and 0.843;when the evaluation results were compared with binary variables,the kappa coefficient was 0.745.The results of test-retest reliability showed that the kappa coefficient and weighted kappa coefficient of retest evaluation results were 0.863 and 0.892.When the evaluation results were compared with binary variables,the kappa coefficient was 0.857.2.Through the collation and analysis of qualitative research data,four themes reflecting the experience of clinical nurses using Chinese version of PURPOSE-T were finally extracted: attitude experience;behavior experience;cognitive experience;and application suggestions.Discussion 1.The Chinese version of PURPOSE-T has good reliability and validity,and can be applied to the risk assessment of stress injury in hospitalized patients in China.The items of the scale are clear and the assessment is convenient.It provides a measuring tool for scientifically and effectively assessing the risk of pressure injury.2.The Chinese version of PURPOSE-T has been widely accepted and approved by clinical nurses in clinical application.The items of the scale are clear,the risk factors are comprehensive,and the assessment is simple.Detailed skin assessment items have been unanimously praised.The application experience of clinical nurses provides scientific and practical guidance for the clinical application of Chinese version of PURPOSE-T. |
