Study On The Relationship Between Job Stress, Job Burnout And Organizational Commitment In ICU Nurses

Objective:To explore the current situation and analyze the relationship of ICU nurses’job stressor, job burnout and organizational commitment, and present some effective suggestions for nursing managers to decrease nurses’job stress, stimulate their work effectiveness, improve nursing quality and increase patients’satisfaction.Methods:233nurses working in5comprehensive hospitals of Jinan city of Shandong Province were selected as a sample by cluster random sampling method. We used the Maslach Burnout Inventory, Nursing Job Stressor Inventory, Organizational Commitment Questionnaire and demographic data in the research. The data was analyzed by SPSS13.0.Results:1. The mean score of ICU nurses’job stressor was2.11±0.37.All the job stressor in descending order were professional and career issues, workload and time pressure, patient care, resource and environmental problems, management and interpersonal relationships issues. The findings showed that work stressor was negatively correlated with organizational commitment (r=-0.33,p<0.01)2. The mean score of ICU nurses’job burnout was2.11±0.82. The results showed that94.40%of the subjects had a high level of emotional exhaustion,28.80%had a high level of depersonalization,0.40%had a high level of reduced personal accomplishment. ICU nurses’job burnout level was negatively correlated with organizational commitment (r=-0.22, p<0.01)3. The mean score of ICU nurses’ organizational commitment was4.56±3.27; of which the dimension of affective commitment was3.56±0.69; of the continuance commitment was3.03±0.60; of the normative commitment was3.21±0.66。4. ICU nurses’ organizational commitment was influenced by the characteristics of social demography. Different positions and employment status had significant impact on normative commitment (p<0.05), and different salary level had significant influence on continuance commitment (p<0.05)5. Analysis of multiple linear regression between influential factors and organizational commitment showed that management issues and interpersonal relationships and decreased personal accomplishment had significant impact on ICU nurses’ affective commitment; management issues and interpersonal relationships, emotional exhaustion and decreased personal accomplishment had significant impact on continuance commitment; management issues and interpersonal relationships, professional and career issues and emotional exhaustion had significant influence on normative commitment.Conclusions:1. ICU nurses’ job stress was in a higher level in general, especially in the facet of professional and career issues. Hospital mangers should pay attention to the causes of nurses’job stress, improve their salary benefit, provide opportunities for promotion, reduce some paperwork and non-nursing work, respect nurses, et al.2. Among ICU nurses’ job burnout, the emotional exhaustion brought from work was in a high level, the depersonalization was in a moderate level, while the reduced personal accomplishment was in a low level. This indicated that prevention on nurses, including learn interpersonal communication skill and self-regulation, could relieve the pressure.3. ICU nurses’ organizational commitment was in a high level, of which the affective commitment scored the highest. The normative commitment was influenced by positions and employment status, and the continuance commitment was affected by salary level. 4. ICU nurses’ organizational commitment was negatively correlated with job stressor and job burnout. Management issues and interpersonal relationships could predict all the latitudes of organizational commitment, which means the harmonious interpersonal relationships and better work atmosphere could enhance ICU nurses’ commitment to the organization. Decreased personal accomplishment had a predictive impact on affective commitment and continuance commitment, while emotional exhaustion had a predictive effect on continuance commitment and normative commitment. These mean work-related accomplishment and better emotional experience could influence nurses’commitment.
